Embodiment 1

[0086] METHODS: Fresh bovine bladders were used to create two models of the ureteral hernia wall. The first is a herpes model created by injecting 1-2 mL of stained standard saline into the submucosa. The second is the exfoliated mucosa that separates from the muscle layer. The tissue is located in a degassed water bath, and the target layer is brought into focus with a 1 MHz ultrasound transducer. Pulsed focused ultrasound is applied to create visible holes in the wall. The pulse amplitudes employed were similar to those applied for extracorporeal shock wave lithotripsy, with a peak positive pressure p+=100-120 MPa and a peak negative pressure of 17-20 MPa. Pulse duration and pulse rate were varied between different exposures. Record the puncture time and puncture size. The use of ultrasound imaging as targeting and processing feedback has also been explored.

[0090] In this example, a tissue model of the ureterocele wall was used to evaluate the feasibility of using tissue abolition under ultrasound image guidance to produce mechanical punctures.

[0091] Materials and methods: Freshly resected bovine bladder tissue was used to model the ureteral hernia wall. Bladders were harvested and kept in degassed phosphate buffered saline until use (less than 12 hours from resection). The bladder was divided into 5x5 cm segments. The mucosa and submucosa are stripped from the underlying muscle and adventitia to form a membrane with a thickness of 0.5-1 mm. Mucosa is placed over the circular opening of a polypropylene chamber containing stained saline. The membrane is secured around the container by straps such that the membrane seals the fluid within the chamber without exerting tension on the membrane, other than that caused by the sample's own weight. Abstract

Methods for diagnosing a pathologic tissue membrane, as well as a focused ultrasound apparatus and methods of treatment are disclosed to perform ureterocele puncture noninvasively using focused ultrasound-generated cavitation or boiling bubbles to controllably erode a hole through the tissue. An example ultrasound apparatus may include (a) a therapy transducer having a treatment surface, wherein the therapy transducer comprises a plurality of electrically isolated sections, (b) at least one concave acoustic lens defining a therapy aperture in the treatment surface of the therapy transducer, (c) an imaging aperture defined by either the treatment surface of the therapy transducer or by the at least one concave acoustic lens and (d) an ultrasound imaging probe axially aligned with a central axis of the therapy aperture.
