Abstract

According to one embodiment, an image forming apparatus includes an acquisition unit, an issuing unit, an image creating unit, a code creating unit, a first storage unit, and an image forming unit. The code creating unit creates one code image by encoding information including the plural pieces of identification information for the plural images arranged in the image for image formation. The image forming unit forms an image including the image for image formation and the code image on one sheet.
