A vial adapter having a needle-free valve, a sharpened cannula used to perforate a vial's rubber stopper, and a circular array of claws of different lengths to engage vial closures of different diameters. The array of claws includes a first set of claws each having a first length extending inwardly from the periphery of the housing of the adapter and a second set of claws alternating with the first set of claws and each having a longer length. The second set of claws are mounted so that they deflect and plastically deform out of the way in the case where the adapter is engaged with a vial that exceeds a predetermined size. The housing includes a shroud that is at least as long as the sharpened cannula to protect medical personnel who use the adapter from inadvertent punctures. The filter media allows the passage of air to the atmosphere outside the vial for pressure equalization. In another aspect, a third filter medium is used in the vent of the adapter to allow gas to pass in either direction through it, but prevents bacteria and particulate matter in the atmosphere from reaching the second filter device. The base end portion of the needle is provided with a stopper contact portion which contacts the top surface of a stopper when the fitting portion is fitted in the head portion of the vial. An assembly is provided which includes an infusion device coupled to a standard medication syringe. The medication syringe may be coupled to a stopcock valve having multiple ports and to which syringes, vial adapters, infusion tubing, and multiple other items may be coupled. The infusion device includes a source of power based on a resistance force such as vacuum, spring or gas power. The infusion device converts the resistance based force to usable work in the form of a force applicator. The force applicator includes a driver section on one section of a reciprocating arm and an attachment to the power source on another section of the arm. The driver is pulled outward (excursion) to increase the size of the chamber, creating a force that tends to return the driver back inward, causing incursion.
