The invention discloses a torque output mechanism of a ratchet wrench, which comprises a force application unit, a transmission system and a hollow head, wherein one end of the force application unit is provided with two forked support arms, the other end of the force application unit is connected with a rotatable holding unit; the transmission system is embedded inside one of the support arms and comprises a drive wheel, a plurality of driven wheels and a hub unit, the drive wheel receives the power of the holding unit for rotating, the driven wheels are mutually engaged and used for delivering the power of the drive wheel, and the hub unit is connected with one of the driven wheels; and the hollow head is coupled and can swing between the two support arms, a non-circular insertion piece, a clutch mechanism and a direction selecting device are arranged inside the hollow head, the clutch mechanism receives the power of the hub unit to drive the insertion piece for rotating, and the direction selecting device determines the one-way blocking of the insertion piece.
