The form generation device comprises a self-defined template creating module, a form attribute configuration module and a form generation module, wherein the form attribute configuration module comprises a field display name configuration module, a visibility configuration module, a modifiability configuration module and a filling requirement configuration module; the field display name configuration module is used for configuring the displayed name in the same field by a user; the visibility configuration module is used for controlling the visibility of certain fields in a form; the modifiability configuration module is used for controlling the modifiability of certain fields in the form; the fillingrequirement configuration module is used for controlling whether the fields of the form are requiredto be filled, if the fields of the form are required to be filled, outputting a focus event, and checking the required fields item by item when the form is submitted; and the form generation module generates a target electronic form which contains each field configured by the form attribute configuration module. By the form generation device provided by the invention, the attributes can be set for all forms; whether to display, the displayed name, whether the fields can be modified, and whether the fields are required to be filled can be set; and the form can really be configured flexibly.
