An instrument guide assembly for a bone plate includes an insert (3,3') configured to be arranged in a hole (2) of a plate member (1), the insert (3,3') has a through-hole (30), and a guide member (4,4') is detachably arranged in the through-hole (30) of the insert (3,3'), the guide member (4,4') has a head portion (41), a shaft portion (42) extending from the head portion (41), a vertical axis (C) and a guide channel (40) extending through the head portion (41) and the shaft portion (41) for guiding a drill or other instrument therethrough. The insert (3,3') and the guide member (4,4') are used to connect each other.
