A US image 3 of a subject's body region includes tissue to which radiation is simultaneously delivered from a radiation source such as a linear accelerator (LINAC) 6 . The US image 3 is stored in memory, which can include processor memory, computer memory, or persistent storage such as disk storage. Abstract

A radiation therapy system (1), comprising: an ultrasound (US) imaging unit (2), a registration unit (30), a US motion unit (44), and a real-time dose calculation engine (46). The ultrasound (US) imaging unit (2) generates baseline and real-time US images (3) of a region of a subject's body (4) including a target and one or more organs at risk (OAR). The registration unit (30) deformably registers a planning image (32) with the baseline US image (36), and maps (66) radiation absorption properties of tissue in the planning image (32) to the Baseline US images as described (36). The US motion unit (44) measures motion of the target volume and OAR during a radiation therapy treatment based on the real-time US images. The real-time dose calculation engine (46) calculates a real-time radiation dose delivered to the tissue based on the tissue radiation absorption properties mapped from the baseline or planning image to the real-time 3D US image (3).
