[0037] The present disclosure relates to medical devices, and in particular to devices for percutaneous division of fibrous structures. Although the devices and methods described herein may be used to percutaneously segment any kind of fibrous structure in the body, as an exemplary application, the present disclosure may sometimes refer to the treatment of carpal tunnel syndrome. The carpal tunnel is an anatomical compartment in the wrist bounded by the carpal bones and transverse carpal ligament. The clinical symptoms of carpal tunnel syndrome are primarily caused by compression of the median nerve as it passes through the carpal tunnel. Abstract

A device for segmenting a fibrous structure comprising: a catheter; an expandable member positioned near a distal end of the catheter and in fluid communication with a lumen of the catheter; and a Cutting elements on the outer surface. A method for dividing a fibrous structure, comprising: positioning an expandable member having a cutting element disposed thereon adjacent to the fibrous structure; expanding the expandable member outwardly to tension the fibrous structure across the cutting element; and activating the cutting element to weaken or cut the fibrous structure. A method for treating carpal tunnel syndrome comprising: inserting a needle into the carpal tunnel; guiding a guide wire to a position adjacent to the transverse carpal ligament; advancing a device having an expandable member and a cutting element along the guide wire; positioning the a cutting element; tensioning the ligament across the cutting element; and weakening or cutting the ligament.

In certain pathological conditions, structures within the compartment may become compressed due to swelling or inflammation of the structure or contraction of the compartment walls.
