In some aspects, a helical coil fastener and associated applicator tool and surgical insertion method are provided to attach the IOL to the iris relative to the pupil. In one embodiment, the IOL assembly includes a helical coil fastener to attach the IOL to the iris to correct astigmatism, presbyopia, and / or nearsightedness or hyperopia. An IOL can be implanted to correct vision errors with the patient's natural lens removed or left in place. The IOL can be used as a phakic implant (eg, for use with a phakic) or as an aphakic implant (eg, for use without a phakic).
