The present invention is directed to a novel device and method for providing a disposable endotracheal intubation device for use with an auxiliary passageway serving as a guide for the placement of an orogastic or other enterally directed device in a patient. The present invention pertains to a combination intubation device comprising an endotracheal tube and a catheter proximate the endotracheal tube to guide the path of an enteral tube. In a preferred embodiment, the endotracheal tube is capable of defining an arcuate path in a first geometric plane between its proximal end and its distal end to facilitate introduction of the tube into the trachea of a patient. In a preferred embodiment, the catheter employs a fenestration to facilitate removal of the intubation device from the patient without removal of any enteral device previously directed therethrough into the patient.

The introducer has a tubular wall that defines a lumen extending between a proximal end and a distal end of the introducer. The tubular wall has an outer diameter that is less than an inner diameter of the endotracheal tube. The tubular wall is circumscribed by an invertible shroud. The invertible shroud flexes distal-ward (“forward”) proximal-ward (“rearward”). The proximal end of the introducer is introduced into a distal end of the endotracheal tube and the shroud is manually retroflexed rearward to cover the sharp margins of the end of the endotracheal tube prior to its insertion into a patient's airway. After the endotracheal tube has been properly positioned, the introducer is withdrawn, the motion of its withdrawal anteflexing the shroud forward for removal through the endotracheal tube.

A bite block for an endotrachial tube includes a hollow body extending between open ends to receive an endotrachial tube such that opposing ends of the tube protrude through the open ends of the bite block. A device for biofilm removal from an endotracheal tube is disclosed. Such a device may include a hollow catheter having a tip, wherein said device is inserted down an endotracheal tube of an orally intubated patient. Said tip may include a cylindrical central portion and a fin configured to dislodge biofilm from the inner lumen of the endotracheal tube as the device is rotated, either manually or mechanically, inside the endotracheal tube. The fin may be configured as a helix and positioned to spiral around said cylindrical central portion of the tip thereby forming a channel, defined by the pitch of said helix, about the circumference of the cylindrical central portion. Biofilm dislodged from the inner surface of the endotracheal tube by the rotational motion of the fin may be retained within the channel until the tip is removed from the endotracheal tube. The device may further comprise a motor for mechanically rotating the catheter and tip. Finally, the device may be configured to interface with a mechanical ventilator such that airflow to an intubated patient is not impeded while said device is used to remove biofilm from the inner surface of the patient's endotracheal tube.
