An identification tag and related tag system are provided for securely mounting the identification tag onto a selected wearer or object, such as mounting onto a wearer's wrist or the like. The identification tag is adapted to receive wearer-related identification and other information in human readable and/or machine readable form, and for slide-fit assembly onto an elongated flexible strap such as a wristband or bracelet for mounting onto the selected wearer or object. In one preferred form, the identification tag includes a radio frequency identification (RFID) circuit adapted for communicating wearer-related information with a remote reader. The tag system, including the identification tag assembled with the flexible strap, is particularly useful with a small wearer or object, such as an infant, to provided a relatively extensive and relatively flat information-bearing surface area.

An identification bracelet for mounting about a person's wrist or the like incorporates a sealable window to protect wearer-related information against contact with moisture and the like. The bracelet includes a flexible band defining an information-bearing zone, in combination with an overlying adhesive-backed transparent cover strip. In an initial state, the cover strip has opposite ends securely adhered to the band, and a central window segment separated from the band by a peel-off release film. The window segment is adapted for lift-away separation from the band as by tearing along a line of weakness line at one end thereof to expose the information-bearing zone for receiving wearer-related information, and for facilitated peel-off removal of the release film, followed by adhesively seating the strip central window segment onto the band in a manner defining a sealed perimeter overlying and protecting the wearer-related information.

An improved identification band such as a wristband is provided for mounting onto a specific person or object. The improved identification band includes a primary identification zone adapted to receive human-readable and/or machine readable information as by printing and/or programming of a radio frequency identification (RFID) circuit. The band further includes multiple detachable adhesive labels forming a portion of the band or alternately forming a tail end extension thereof, wherein each label is adapted to receive human-readable and/or machine readable information as by printing and/or programming of a radio frequency identification (RFID) circuit. In use, such as when worn by a patient in a hospital or the like, the detachable labels are individually separable from the identification band on an as-needed basis for adherence to other objects associated with the band wearer, such as a vial containing a patient blood sample or the like.

A business form particularly adapted for use during medical emergencies includes in a first embodiment a wristband/label assembly that is readily separable from a carrier, with the wristband including a single end for looping around a victim's appendage through a cinch and a tab carrying a plurality of labels with the wristband and each label having an identifying indicia such as a bar code printed thereon. The wristband bar code thus becomes associated with the victim and the labels are used to identify items associated with the patient such as his possessions, medical charts, medicines, etc. The wristband may be color coded so that as the medical personnel triage victims they are categorized by color as to their need for medical care, with the color coding thus being readily ascertainable by others as multiple victims are processed.
