Problems solved by technology

However, this known system offers only limited possibilities for imaging in order to determine the proper implantation position and the catheter guidance to this position
In particular, the spatial correlation between the coronary venous system and the left ventricle was unknown during the design of this routine procedure

Embodiment Construction

[0014] The method and system of the present invention allow for the acquisition of computed tomography (CT) images of the pulsating heart during the insertion process of the electrodes used to implant the biventricular pacing system. The method and system can be directly integrated into an existing angiography system, for example, the existing angiography system can be purchased from Siemens Medical Systems AXIOM Artis system.

[0015] In step 1 shown in Figure 1, a 3D data set representing the left ventricle is acquired. This left ventricle will then be equipped with the three electrodes of the biventricular pacing system. In order to obtain the 3D data set, the contrast agent can either be injected directly into the ventricle or intravenously into the ventricle. The resulting image is schematically illustrated in Figure 2A.

[0016] Alternatively, the function of the left ventricle can be judged by using the image of the left ventricle shown in FIG. 2A.

Abstract

Obtaining a 3D image of the left ventricle and obtaining a coronary sinus vascular tree of a heart in a method and system for supporting implantation of a third electrode of a biventricular cardiac pacing system at an optimal location in the coronary sinus vascular tree 3D images, and these images are combined and displayed. The physician electronically interacts with the displayed image to indicate a marker on the image at a target location in the coronary sinus vascular tree for securing the third electrode. During implantation of the third electrode, a real-time image of the vascular tree of the coronary sinus is acquired, and the combined image with the markers therein is superimposed on the real-time image. Using the real-time image with markers superimposed on it, the physician uses the implant tool to guide the implantation of the electrodes to the selected target location.

Description

Technical field [0001] The present invention relates to a method and system for supporting the implantation of a cardiac pacemaker, in particular for supporting the implantation of a biventricular cardiac pacemaker. Background technique [0002] In clinical medicine, conventional pacemakers can be used to treat tachycardia and bradycardia. In contrast, biventricular pacemaker therapy, also known as cardiac resynchronization therapy (CRT), is used to artificially generate synchronization between the left and right ventricles, which is not the case in patients with special types of cardiology or cardiac insufficiency. This synchronization exists or does not always exist. Three electrodes are required for biventricular pacing. Typically, two of these electrodes are placed in the right atrium and right ventricle where conventional pacemaker electrodes are normally placed. The third electrode in biventricular pacing is placed in a side branch of the coronary venous tree. To this end, ...
