Embodiment

[0139] Example I

[0140] Exemplary devices and methods described in this disclosure (eg, as Figure 3-8 shown in ) was used in a series of experiments in porcine lungs with EVLP and combined negative and positive pressure ventilation (NPV / PPV). For each experiment, a pair of lungs were rapidly excised from 40-50 kg pigs after appropriate euthanasia and exsanguination. Mount the lungs in a sealable container in this device. An endotracheal tube is attached to the windpipe of the lungs. The vasculature of the lung is connected to the perfusion system and begins to perfuse. The perfusate includes decellularized albumin solution, a mixture of whole blood and albumin solution, or a mixture of packed red blood cells (pRBC) and albumin solution. The composition of the albumin solution is listed in Table 1.

Embodiment IV

[0167] Embodiment IV (contrast)

[0168] Figure 22 An exemplary control device 400 for negative pressure ventilation is illustrated and tested with three porcine lungs (N=3). Device 400 is used to apply negative pressure within organ chamber 410 . In this example, device 400 is not used to deliver positive airway pressure in the lung airways.

[0169] In testing, a donor lung 450 is placed in a hard shell sealed container 410 in a device 400 . Container 410 is partially filled with saline liquid 480 . The lungs 450 float on top of a flexible plastic membrane 482, and the liquid 480 keeps them afloat.

[0170] Tracheobronchial tree 452 of lung 450 is connected to conduit 432 via endotracheal tube 444 . A catheter 462 connects the perfusion device 460 to the pulmonary artery 454 of the lung 450 . Conduit 464 connects perfusion device 460 to pulmonary vein 456 of lung 450 .

[0171] The container 410 is sealed from the atmosphere by a lid 412 . Conduit 432 extends throug...

Abstract

The present invention relates to a method of ventilating excised lungs. A ventilation gas is supplied to an airway of a lung and a vacuum is formed around the lung. A quality of the vacuum is varied between a lower level and a higher level to cause the lung to breathe, while the pressure of the ventilation gas supplied to the airway is regulated to maintain a positive airway pressure in the airwayof the lung. The vacuum may be cyclically varied between the two vacuum levels. The levels may be maintained substantially constant over a period of time, or one or both of the lower and higher levels may be adjusted during ventilation. The lung may be placed in a sealed chamber, and a vacuum is formed in the chamber around the lung.

Description

[0001] Cross References to Related Applications [0002] This application claims priority and benefit to US Provisional Patent Application Serial No. 62 / 343,076, filed May 30, 2016, the entire contents of which are incorporated herein by reference. technical field [0003] The present disclosure relates generally to devices and methods for lung ventilation, and more particularly to devices and methods for ventilation of resected lungs by varying external pressure. Background technique [0004] In order to use excised donor lungs for transplantation, the excised lungs may need to be perfused and ventilated ex vivo prior to the transplant procedure to restore or maintain their function, or to assess or evaluate their potential for transplantation. quality or suitability. [0005] For ex-vivo ventilation of excised lungs, a common traditional mechanical ventilation technique utilizes positive pressure applied to the tracheobronchial tree. This creates a pressure gradient betw...
