The invention discloses a prefabricated UHPC central anti-collision guardrail grouting connection structure which comprises a main beam top plate provided with connecting steel bars and a UHPC anti-collision guardrail, wherein grouting sleeves are separately pre-embedded on the inner left and right sides of the bottom of the UHPC anti-collision guardrail; at least three internal shear connectors are distributed at intervals in the longitudinal direction on the inner wall of each grouting sleeve; external shear connectors corresponding to the internal shear connectors are distributed at intervals in the longitudinal direction on the outer wall of each grouting sleeve; the grouting sleeves are provided with openings for inserting the connecting bars into pouring chambers; and the grouting sleeves are provided with grout inlets positioned at the lower part and grout outlets positioned at the upper part. Grouting materials are poured into the pouring chambers to connect the UHPC anti-collision guardrail with the main beam top plate; the connection between the grouting sleeves and the UHPC anti-collision guardrail is achieved by virtue of the external shear connectors; and the groutingsleeves and the main beam top plate are connected into integration through the cooperation of the internal shear connectors, the connecting steel bars and the grouting materials.
