Capsulotomy is a type of eye surgery in which an incision is made into the capsule of the crystalline lens of the eye. In modern cataract operations, the lens capsule is usually not removed. The most common forms of cataract surgery remove nearly all of the crystalline lens but do not remove the crystalline lens capsule (the outer “bag” layer of the crystalline lens). The crystalline lens capsule is retained and used to contain and position the intraocular lens implant (IOL).

The method includes performing lens fragmentation of a lens in the capsular bag to yield lens material, inserting a first instrument into the first incision, inserting a second first instrument into the second incision and removing the lens material via one of the first instrument and the second instrument and through one of the first incision and the second incision. The tensile structure of the anterior portion of the capsular bag is maintained such that accommodation exists within the eye after insertion of the intraocular lens.

Embodiments of the invention provide hydrodissection and / or PCO prevention or reduction in a patient undergoing eye surgery.
