The body includes a base portion and a sleeve portion, the base portion being dimensioned to engage the skin of a patient and having a transverse bore, the sleeve portion extending upwardly from the base portion and having a longitudinal slot aligned with the transverse bore and a transverse slot intersecting the longitudinal bore. The clamp, which is slidably mounted on the base portion and across the transverse slot of the sleeve, comprises a plate having a transverse opening. The transverse opening has a wide region and a narrow region, the two regions being alternately alignable with the longitudinal bore to open and to close, respectively, the feeding tube. The feeding tube has a distal end adapted to be anchored to the inside of a patient and a proximal end inserted up through the base portion and the sleeve portion, including the transverse opening of the clamp situated within the sleeve, and then inverted over the top edge of the sleeve. The cap is then mounted on top of the sleeve so as to secure the inverted end of the catheter to the exterior of the sleeve. The cap is provided with an opening through which access to the catheter may be gained. An inner sleeve, sized to engage the inside surface of the feeding tube, is removably inserted through the cap and the feeding tube, the inner sleeve having a proximal end to which a tubular fitting is secured. Food and / or medications are dispensed to the patient through the fitting and the inner sleeve and, in this manner, prevent clogging of the feeding tube.

Surgical fastening tool

A surgical tool is disclosed. The tool is found useful in performing percutaneous endoscopic gastrostomy and other surgical procedures requiring formation of a stoma into a body lumen. Such a tool would contain a first and second cannulae concentrically nested within one another and secured together at a distal end. An actuator is provided for selectively engaging and disengaging a retention mechanism located on that portion of the tool positioned within the body lumen. A locking mechanism is also provided to be used in conjunction with the retention mechanism for securing the cannulae to one another enabling the cannulae to be severed, and the actuator to be disposed, until the locking mechanism is disengaged upon formation and healing of an artificial stoma.

In one embodiment, the assembly is a low profile percutaneous endoscopic gastrostomy (PEG) device and comprises a body, a clamp, a feeding tube, a cap and an inner sleeve assembly. The body includes a base portion and a sleeve portion, the base portion being dimensioned to engage the skin of a patient and having a transverse bore, the sleeve portion extending upwardly from the base portion and having a longitudinal slot aligned with the transverse bore and a transverse slot intersecting the longitudinal bore. The clamp, which is slidably mounted on the base portion and across the transverse slot of the sleeve, comprises a plate having a transverse opening. The transverse opening has a wide region and a narrow region, the two regions being alternately alignable with the longitudinal bore to open and to close, respectively, the feeding tube. The feeding tube has a distal end adapted to be anchored to the inside of a patient and a proximal end inserted up through the base portion and the sleeve portion, including the transverse opening of the clamp situated within the sleeve, and then inverted over the top edge of the sleeve. The cap is then mounted on top of the sleeve so as to secure the inverted end of the catheter to the exterior of the sleeve. The cap is provided with an opening through which access to the catheter may be gained. An inner sleeve, sized to engage the inside surface of the feeding tube, is removably inserted through the cap and the feeding tube, the inner sleeve having a proximal end to which a tubular fitting is secured. Food and / or medications are dispensed to the patient through the fitting and the inner sleeve and, in this manner, prevent clogging of the feeding tube.

A PEG catheter is prevented from being infected. An infection preventive cover includes an elongated sheath having one opening end. The opening end is embroidered with a ligature at its circumferential section. A guide wired which is inserted through an abdominal wall into a stomach and which is then drawn out of an oral cavity is linked with a joint wire of a PEG catheter. Thereafter, the PEG catheter is covered with the infection preventive cover. The opening end of the sheath is tied up by the ligature. By pulling the guide wire on the abdomen wall side, the PEG catheter covered with the cover is delivered into the stomach. By pulling a cutting wire exposed through the abdomen wall, the ligature is cut off and the opening end of the cover is opened. Further in order to make it easy to close tightly and to loose an opening end, another infection preventive cover is provided. A closing thread which is embroidered along a circumferential edge of the opening end of a sheath is apart from the embroidered portion and loosely passes through a tightening tube to be led outside from an outer end of the tube. when a portion of the thread which is led outside the tube is pulled out, the opening end of the sheath is closed by the closing thread. The thread is kept in its tightened state. When the tightened thread is loosened, the opening end of the sheath is naturally opened or becomes openable state.
