The system is characterized by comprising a platform server, a plurality of medical care terminals, a patient monitoring device and a data server. A data acquisition part is used for acquiring body monitoring information containing multiple parameters from patients according to a preset acquisition frequency. A monitoring-side communication part is used for sending the body monitoring information and corresponding patient identification information to the platform server. A patient data control part is used for controlling a monitoring information receiving part to receive the body monitoring information sent from the patient monitoring device. A control parameter screening part is used for screening out and classifying out the parameters in the body monitoring information to serve as disease condition parameters according to a disease condition scoring method. A control disease condition calculation and evaluation part isused for calculating a current disease condition risk value and evaluating a disease condition level according to the disease condition scoring method and the disease condition parameters. A controlrisk calculation and evaluation part is used for calculating an overall risk value and evaluating a risk level according to a plurality of disease condition risk values.
