The three-dimensional positioning device comprises an arc-shaped substrate, wherein the arc-shaped substrate is provided with a first cylinder, a second cylinder and a third cylinders, which are parallel; and free ends of the first cylinder, the second cylinder and the third cylinder are connected into a right-angled triangle. The method comprises the following steps that CT (Computed Tomography) scanning is carried out on the three-dimensional positioning device, a standard three-dimensional model of the device is reconstructed, a patient wears a radioactive guide plate with the three-dimensional positioning device for taking CT, data are imported into implantation software for simulating implantation, the standard three-dimensional modeling of the device is registered into the CT of the patient, space measurement is carried out on an implantation body on the basis of the standard three-dimensional model of the device, three-dimensional relation of the implantation body and the jawbone is accurately transferred onto an entity model, and then the oral implantation guide plate is manufactured by a mechanical perforating technology. The method is simple and convenient in operation, does not need to be dependent on special equipment, does not consume materials basically, is economic and practical, can be finished by independent operation of a doctor or a technician and has a good clinical application prospect.
