Problems solved by technology

[0003] Changing multiple instruments complicates endoscopic procedures and increases the likelihood of contamination each time one of the instruments is swapped out
For example, multiple instrument changes increase the risk of peritonitis due to bile leaks
Additionally, complications may arise due to leaks during instrument changes, bleeding, and guidewire migration

Embodiment Construction

[0029] The disclosed embodiments are designed to address the above-mentioned problems. In particular, the disclosed embodiments include devices that can be used to access and form pathways in the walls of hollow bodily structures such as organs, lesions, cysts, pseudocysts, and abscesses. This device can be used to create access for all EUS drainage procedures from puncture to dilation. For example, the device may form a pathway from the stomach to the intrahepatic bile duct, from the stomach to the pancreatic duct, from the stomach to a pancreatic cyst, from the duodenum to the common bile duct, from the duodenum to the gallbladder, and the like. The device can alternate between piercing and dilating without having to interchange between internal therapy tools due to complete removal of one device for another, reducing the risk of bile leakage when changing tools. Additionally, the disclosed embodiments provide a more stable device that prevents inadvertent movement of the i...

Abstract

The present invention relates to a device for insertion into a body through a working channel of an endoscope, comprising: a catheter comprising a dilator; a guide tube disposed in the lumen of the catheter; and a handle including a puncture actuator operably coupled to the proximal end of the guide tube. The dilator may be a cautery device and / or a balloon. The device may also include a stylet including a cuttable distal end for puncturing tissue and extending through the lumen in the guide tube. The handle may further include a stopper detachably coupled to the puncture actuator to fix a position of the puncture actuator on the handle, and the stopper is movable on the handle independently of the puncture actuator when disengaged from the puncture actuator. The disclosed embodiments also include a method of forming a pathway in a hollow body organ wall using the apparatus.

Description

Background technique [0001] Endoscopic ultrasound (EUS) surgery is a minimally invasive procedure that provides access to organs, lesions, cysts, pseudocysts and abscesses close to the gastrointestinal tract for diagnostic and therapeutic purposes. Interventional EUS is frequently used for drainage of pancreatic juice collection, biliary and pancreatic duct drainage, gallbladder drainage, and other conditions, including in cases of failed endoscopic retrograde cholangiopancreatography. [0002] But EUS involves multiple steps and instruments. For example, EUS typically involves advancing one or more puncture and / or dilation devices through the working channel of an ultrasound endoscope to puncture target tissue and dilate the punctured pathway to provide access for the introduction of drainage or other devices. EUS is a long and cumbersome procedure involving multiple steps and the replacement of multiple instruments such as needles, stylets, guide wires, catheters, and the l...
