Embodiment 1

[0018] A 52-year-old male suffered from nephrotic symptoms indicated by proteinuria and red blood cells in the urine in 1982. After renal biopsy and histological analysis of kidney tissue, he was diagnosed with IgA nephropathy. He was treated with various antihypertensive drugs, but proteinuria continued to increase and by 1990 he had developed renal insufficiency, and after that year he developed uremia which itself required dialysis. In 1993 he received a kidney from a deceased person. The transplanted kidney functioned satisfactorily for the first 24 months and the patient was treated with a systemically acting glucocorticoid substance (prednisolone) and an immunosuppressant drug (cyclosporine A) . In 1995, the first signs of nephropathy in transplanted kidneys were detected based on an increase in proteinuria as measured by changes in serum creatinine concentration and a decrease in renal function. After performing a renal biopsy of the graft and subsequent histological...

Abstract

The invention provides the use of a glucocorticoid having a first pass metabolism in the liver of at least 90 % as active substance, for the manufacturing of a medicament for oral or rectal administration in the treatment of glomerulonephritis by releasing the active substance in the intestine. The invention also provides a method for treatment of glomerulonephritis in a native kidney or a kidney transplant with the glucocorticoid as defined above. The invention also comprises a composition comprising the active substance and a pharmaceutically acceptable carrier, adjuvant or diluent designed for oral or rectal administration.

Description

field of invention [0001] The present invention relates to a method and means for treating glomerulonephritis. Background of the invention [0002] Functional units of the kidney, such as glomeruli, can suffer from inflammation. Inflammation that occurs in the glomeruli is called glomerulonephritis and it can be classified into subtypes such as membranous glomerulonephritis, focal segmental glomerulonephritis, diffuse mesangial hyperplastic nephritis Glomerulonephritis, intracapillary or extracapillary proliferative glomerulonephritis. Using histopathological techniques, these subtypes vary with microscopic or immunohistochemical photographs. One cause of inflammation is the result of deposition of immunoglobulin A (IgA) in the glomeruli. This condition is called IgA nephropathy (1-3) and in a generally correct view it is the most common form of glomerulonephritis. [0003] The assessment of the severity of glomerulonephritis is based on the results of different studies....
