A patient interface to provide breathable gas to a patient, comprising a cushion assembly including a nasal cushion having a nasal opening to direct breathable gas to a patient's nares; a nasal plenum; an oral cushion to contact and seal against a patient's face around a patient's mouth; an oral plenum to form an oral air chamber with the oral cushion, the nasal plenum and the oral plenum being pneumatically connected; an anterior opening positioned opposite the oral opening to receive breathable gas; a positioning and stabilizing structure including an upper strap, a lower strap, and a clip; and a single plate member including a connection port to receive breathable gas from a gas delivery tube and direct it to the cushion assembly, an upper portion having upper attachment members, and a lower portion having lower attachment members, each upper strap connected to a corresponding upper attachment member, each lower strap connected to a corresponding clip, each clip connected to a corresponding lower attachment member, the upper portion of the single plate member contacting the nasal plenum to support the nasal cushion against the patient's face.
