A business form particularly adapted for use in a medical or hospital environment includes in a first embodiment a wristband assembly that is readily separable from a carrier comprised of a layer of face stock and a laminate, with the wristband including a printable face stock region die cut into the face stock and a strap portion, a laminating portion, and a cinch die cut into the laminate layer. The laminating portion includes two halves which fold together about a fold line to enclose the face stock, with the strap portion extending from one of said halves. The cinch may be located in one of two locations, either in an extension from the other side of the face stock or intermediate the face stock and the strap portion. In either case the cinch comprises a slot through which the strap portion is inserted and then adhered to itself after its length has been adjusted by the medical professional. If an extension is provided, it may be folded over to adhere to the strap portion and clamp it in place there as well as being adhered at its end back onto itself, as described. An extender is also described which includes a clamshell joinder and a tail portion. The clamshell portion may be applied to the strap portion anywhere along its length to extend the effective length of the strap portion. Any of the wristband constructions may be provided on a page sized sheet along with a plurality of self adhering labels, in any of a number of configurations, to suit any particular application, as desired by a user.

Wristband/label assembly business form and method

A business form particularly adapted for use during medical emergencies includes in a first embodiment a wristband / label assembly that is readily separable from a carrier, with the wristband including a single end for looping around a victim's appendage through a cinch and a tab carrying a plurality of labels with the wristband and each label having an identifying indicia such as a bar code printed thereon. The wristband bar code thus becomes associated with the victim and the labels are used to identify items associated with the patient such as his possessions, medical charts, medicines, etc. The wristband may be color coded so that as the medical personnel triage victims they are categorized by color as to their need for medical care, with the color coding thus being readily ascertainable by others as multiple victims are processed. A second embodiment includes a pre-printed form having a tab portion with the bar code labels as in the first embodiment and also a series of tear off tabs for indicating the medical condition of the patient. Additionally, the medical condition tabs may also be bar coded so that the patient's ID and medical condition may both be “swiped” into a data base using bar code information. Once the data is collected, it is conveniently input into a computer with the computer then transmitting the information to a server for display at a web site. The server and related software is fully capable of handling input from multiple computers in real time so that victim information is made available over the internet almost immediately as the victims are processed.

Wristband/cinch with inboard label assembly business form and method

A business form particularly adapted for generalized use, although it may be used in a medical or hospital environment includes in a first embodiment a wristband that is provided with a laminate backing including a tab portion at the opposite end of a free end, with the tab portion having a cinch slot through which the free end is inserted for securing the wristband about a person's appendage. Several self adhering labels are provided inboard of the tab and may be individually separated from the wristband. The labels and the wristband may each be imprinted with identifying indicia such as bar coding. A second embodiment of the invention includes a page sized form with one or more self laminating wristbands readily separable from a carrier with each wristband comprised of a layer of face stock and a laminate, with the wristband including a printable face stock region die cut into the face stock and a strap portion, a laminating portion, and a cinch die cut into the laminate layer. The laminating portion includes two halves which fold together about a fold line to enclose the face stock, with the strap portion extending from one of said halves. The cinch comprises two slots which may both be located outboard of the face stock, or on both sides of the face stock. The free end of the strap may be adhered in place by folding over one of the slots, or by adhering a patch of adhesive carried at the tip of the free end. The face stock may be extended to the edge of the cinch slot to shield the strap from contacting adhesive. Any of the wristband constructions may be provided on a page sized sheet along with a plurality of self adhering labels, in any of a number of configurations, to suit any particular application, as desired by a user.

In other words, the laminating portion includes two halves which fold together about a fold line to enclose the face stock as before, except that the halves fold over end-to-end. The cinch comprises two slots which may both be located outboard of the face stock, and in the laminate portion alone or in both the face stock and laminate portion. Any of the wristband constructions may be provided in any combination, with or without labels, on a sheetlet or a page sized sheet, in any of a number of configurations, to suit any particular application, as desired by a user.

A cushioned wristband includes a carrier having a multi-layered band portion and a strap sewn to the back of the band portion of the carrier. The band has a surface with loop material and the strap has a surface of hook material, the hook and loop surfaces being intended to secure the band in place as it is wrapped about a patient's appendage. The strap is appropriately sized to insert through one or both of two cinch slots of a self-laminating hang tag which may be printed with patient information and separated from a sheetlet or page sized business form processed through a printer for imaging of patient information thereon. The band portion of the carrier includes a cushioned layer of soft foamy or spongy material for contacting a patient's skin to thereby substantially eliminate any possibility for abrasion, rash, or other irritation or injury to the patient through wearing of the wristband.

An author-created template has HTML and OLE components (controls) placed on a page to define a visual representation of a class of object that is part of a database. The controls merge data from a selected current object of the database into the template, producing a visual representation of the object that is both viewable and printable. An extension to the present invention involves the use of multiple-page HTML to dynamically provide subsequent pages based on retrieved data. For example, if the data retrieved by the controls for an invoice is too large to display on a single invoice page, another HTML page, that is different from the first page of the invoice, would be automatically loaded as a template and the overflow data properly merged therewith.

Special precautions self-laminating wristband business form and method

A preferably two ply, page sized business form particularly adapted for use in a medical or hospital environment includes a wristband that is die cut thereinto with a face stock area defined in the top or face stock ply and an overlying laminate portion defined in the underlying laminate ply with one or more cinch slots and a tab through which the strap end of the laminate portion is inserted for securing the wristband about a person's appendage. The business form preferably also includes a plurality of “special precaution” condition labels die cut into the laminate ply for selective separation therefrom so that one or more may be adhered to the face stock imaging area either before or after lamination to warn of a “special precaution” for the wristband wearer. The laminate portion, the label, and the strap end may be color coded to provide a visual indication of this special precaution status. The business form may be provided in sheetlet size, or full page size, one or more wristbands per form. Labels and cards may be die cut into the form for separation therefrom and application to associated medical charts, or other records to provide the same special precautions indication in other parts of a patient's file / records.

A business form such as a wristband or label includes an imaging area of face stock overprinted with a fluorescent ink for receiving a printed bar code to enhance the scannability of the bar code. The ink may be applied by overprinting to a thickness of between about 0.2 to about 0.5 mils, and even more preferably to a thickness of about 0.2 mils. The overprint ink is also print receptive allowing a bar code to be printed onto it with no deleterious effects. The imaging area of the wristband form may be overlaminated and the bar code scannability will still be enhanced. An alternative embodiment comprises a face stock manufactured to have a finished surface exhibiting a fluorescent effect similar to that achieved by overprinting through mixing in appropriate components as the face stock is made.

A security seal is disclosed which comprises a small, adhesive coated patch of laminate for sealing the joinder of the wristband ends and which has points of weakness such as X's die cut therein so that it separates or destructs upon removal to provide an indication of tampering with the wristband. Also disclosed is an embodiment of the wristband form that is imprinted with identifying indicia for collecting, tracking and administering a patient's blood collection and blood product compatibility.
