A business form particularly adapted for use in a medical or hospital environment includes in a first embodiment a wristband assembly that is readily separable from a carrier comprised of a layer of face stock and a laminate, with the wristband including a printable face stock region die cut into the face stock and a strap portion, a laminating portion, and a cinch die cut into the laminate layer. The laminating portion includes two halves which fold together about a fold line to enclose the face stock, with the strap portion extending from one of said halves. The cinch may be located in one of two locations, either in an extension from the other side of the face stock or intermediate the face stock and the strap portion. In either case the cinch comprises a slot through which the strap portion is inserted and then adhered to itself after its length has been adjusted by the medical professional. If an extension is provided, it may be folded over to adhere to the strap portion and clamp it in place there as well as being adhered at its end back onto itself, as described. An extender is also described which includes a clamshell joinder and a tail portion. The clamshell portion may be applied to the strap portion anywhere along its length to extend the effective length of the strap portion. Any of the wristband constructions may be provided on a page sized sheet along with a plurality of self adhering labels, in any of a number of configurations, to suit any particular application, as desired by a user.

A wristband is assembled from a flexible preferably vinyl carrier and a preferably laminated ID label, the carrier having a panel portion and a strap portion with a snap closure located in a toe extension at the end of the carrier adjacent the panel. A business form handling system includes an image reading device for reading a business form and a terminal apparatus installed at the window for discriminatively deciding whether a read image corresponds to a first species of business form by referencing form species identifying definition information stored previously in a file. When the read image corresponds to the first species of business form, the terminal apparatus cuts an image from a field corresponding to a format inherent to the first species of business form to thereby recognize contents of statement from the cut image and executes processing in accordance with result of the recognition. Unless the read image corresponds to the first species of business form, the terminal apparatus decides discriminatively whether the read image corresponds to a second species of business form by referencing the form species identifying definition information. When the correspondence is found, the terminal apparatus cuts an image from a field corresponding to a format inherent to the second species of business form to thereby recognize contents of statement from the cut image.

A business form particularly adapted for generalized use, although it may be used in a medical or hospital environment includes a roll or continuous construction of a plurality of wristbands, with each wristband being separately formed in a panel comprised of an underlying continuous carrier of a laminating material with a series of face stock patches placed therealong. Each wristband is provided with a face stock imaging area die cut into each face stock patch and a laminating portion die cut into the underlying carrier of laminating material, with the two die cut portions being adhered to each other to form a matrix for ready separation from its panel. The laminating portion has a first area for receiving the face stock portion and a second similarly sized and shaped area for fold over lamination of the first area and face stock portion, along with a pair of straps extending to the opposing sides thereof.

A business form such as a wristband or label includes an imaging area of face stock overprinted with a fluorescent ink for receiving a printed bar code to enhance the scannability of the bar code. The ink may be applied by overprinting to a thickness of between about 0.2 to about 0.5 mils, and even more preferably to a thickness of about 0.2 mils. The overprint ink is also print receptive allowing a bar code to be printed onto it with no deleterious effects. The imaging area of the wristband form may be overlaminated and the bar code scannability will still be enhanced. An alternative embodiment comprises a face stock manufactured to have a finished surface exhibiting a fluorescent effect similar to that achieved by overprinting through mixing in appropriate components as the face stock is made.

A business form according to the present invention including a ply of label stock, a ply of release liner stock, and a ply of message or record bearing stock, may be configured as either a continuous or cut sheet form. The ply of label stock has one side coated with a pressure sensitive adhesive. The ply of release liner stock has a release side that engages with the adhesive on the one side of the label stock. The release liner ply and the label stock ply have edges that are laterally offset from one another so that a narrow longitudinal strip of adhesive is unengaged with the release liner. The ply of message bearing stock is adhered at one edge to the label stock at the narrow longitudinal strip of adhesive. A plurality of cuts is defined by, and extends through, the release liner from the back of the business form, whereby the structural integrity of the business form is enhanced.

The controls merge data from a selected current object of the database into the template, producing a visual representation of the object that is both viewable and printable. An extension to the present invention involves the use of multiple-page HTML to dynamically provide subsequent pages based on retrieved data. For example, if the data retrieved by the controls for an invoice is too large to display on a single invoice page, another HTML page, that is different from the first page of the invoice, would be automatically loaded as a template and the overflow data properly merged therewith.
