Systems and methods for identifying risk of kidney graft failure in a subject are provided. An image of a graft biopsy is obtained, the image representing at least a subset of a plurality of morphological classes including a tissue compartment and inflammation class. The image is inputted into a trained model that identifies, as output, for each morphological class in at least the subset, corresponding objects in the image that fall within the morphological class. For each morphological class, corresponding digital features are generated comprising a first subset of individual feature scores obtained using the objects for the morphological class and a second subset of composite feature scores obtained by combining individual feature scores. Digital features for each morphological class are compared to a reference criterion, thus determining risk of kidney graft failure.

Current approaches in small diameter vascular grafts for coronary artery bypass surgeries fail to address physiological variations along the graft that contribute to thrombus formation and ultimately graft failure. An interlayer drug delivery system can sustain delivery of heparin through the graft with a high degree of temporal and spatial control. A heparin-loaded gelatin methacrylate interlayer sits between a biohybrid composed of decellularized bovine pericardium and poly(propylene fumarate) and UV crosslinking is controlled via 3D printed shadow masks. The masks enable control of the resultant gelMA crosslinking and properties by modulating the incident light intensity on the graft. High doses of heparin have detrimental effects on endothelial cell function. When exposed to heparin in a slower, more sustained manner consistent with the masks, endothelial cells behave similarly to untreated cells. Slower release profiles cause significantly more release of tissue factor pathway inhibitor, an anticoagulant, than a faster release profile.

Chronic Lung Allograft Dysfunction (CLAD) is characterized by a progressive and irreversible decline in lung function affecting half of lung transplant recipients within five years and is the major cause of death contributing to a low median post-transplant survival. Disclosed herein is an optimized airway inflammation gene set (AI2) and AI2 score, as well as RSI, RS2, and RS3 subscores, for use in methods of treating or preventing CLAD, methods of treating or preventing graft failure, methods of treating or preventing transplant rejection, methods of treating or preventing small airway fibrosis, methods of treating or preventing antibody mediated rejection (AMR) and methods of reducing mortality associated with graft failure. Also disclosed are methods of detecting or diagnosing CLAD and methods of predicting transplant rejection.
