Integrated medical software system

A medical management system is provided that integrates all aspects of healthcare provider practice management and managed care, including schedule management, patient registration, insurance information, EMR and billing and collections. The system integrates a central framework module, a scheduling module, a registration module, an account management module and an electronic record module to provide a seamless exchange of information. A clinical module provides template builder which allows the users to define a customized template. The custom and standard templates are used by a document builder to generate documents, such as progress notes and H&P notes, which are retained in the patient's electronic chart. The progress note template allows the clinician to record a patient encounter by presenting the clinician with predefined sections and sentences that are easily completed by the clinician during or after the patient encounter. The templates and documents generated are designed to closely resemble a paper chart. Information gathered during patient scheduling and triage are referenced to pre-populate the templates to avoid having the clinician entering redundant information. The documents are relied upon to automatically generate patient superbills and electronic requests are sent to a claims clearinghouse.

Automated data entry and transcription system, especially for generation of medical reports by an attending physician

Written reports are composed using a terminal during or between patient encounters. Selections made on the terminal invoke standard text segments and a narrative comment is recorded, by dictation or otherwise. After a series of patient encounters, such as at the end of the day, automatically generated draft reports are reviewed, revised and dispatched by the practitioner or staff, in a batch process. The terminal accepting selections and comment can be a wireless Web-capable smart-phone or another terminal running an application to interface with the practitioner. The application presents selections for each of a menu of categories, for example common conditions and maladies encountered in specialized practices. The practitioner distinguishes between normal entries that populate the report with standardized comments (or optionally go unreported), versus noteworthy entries that populate the report with preferably speech-recognized text.

Method and system for customer service process management

A method and system for managing customer service processes for individual customers and populations of customers. In a specific embodiment within the health care field, a clinician creates or updates a draft care plan for a patient using generic or locally-adapted template metadata and tailors the draft care plan to the particular patient. The draft care plan includes a list of specific services (health care interventions) to be provided to the patient. The draft care plan can be routed to members of an inter-disciplinary team for input. Once finalized, workflow processes are instantiated for each intervention on the care plan. An itinerary is created for any required patient encounters, optimized using an algorithm that considers pre-existing process instances. A workflow automation server manages the execution of each workflow process instance, invoking resources according to workflow process specification metadata and workflow task type metadata. Care plans and encounter itineraries are automatically translated to a patient-understandable form and made available to patients online and via printed copy. Clinical practice and outcomes data are analyzed to identify opportunities for improvement of the metadata. The method and system also allows for the selection of a population of patients, the analysis of population data to identify opportunities for interventions (needed services), and the initiation of population interventions through batch updates to individual care plans.

Electronic health record system and method for patient encounter transcription and documentation

A patient encounter documentation and analytics system includes a mobile computing platform and a server-based host platform. A mobile application in tandem with a wireless microphone collects voice signals during a patient-caregiver encounter, transforms the voice signals into audio data files, and uploads the audio data files to the server. A speech recognition software module digitally transcribes the audio data file into text. A text processing module extracts and organizes relevant clinical data based on keyword, key phrase and question / answer analysis. Relevance of words and phrases may be determined in view of, e.g., their presence, frequency and context. A diagnostic decision support module enables the healthcare provider to review the determined clinical information and provide a diagnosis associated with the encounter. A documentation skeleton module extracts diagnosis-specific text components from the transcribed text file and assembles an electronic medical document based on the diagnosis and the diagnosis-specific text components.

Medical billing system and method

A probabilistic medical billing system and method using contextual data and inferential logic for use in screening accuracy of medical bill coding and for presenting results as probabilities or predictions of correctness. The probabilistic medical billing system and method is accomplished using the contextual information contained in a care givers' patient encounter notes, a set of rules and keywords, and an inferential, logic, engine based on Bayesian mathematics or similar disciplines. The inventive device includes an input device to capture care giver's encounter notes or other information, a lexical engine that extracts information while preserving the contextual order of the information, a relational database that contains keywords, phrases and rules and a statistical / probabilistic engine that uses Bayesian mathematics or similar disciplines to create the output. The lexical engine parses a document into words and is capable of extracting keywords or phrases as listed or defined in a master list. Further, the lexical engine would preserve the relative position of discovered keywords or phrases as the keywords or phrases and relative positions were encountered. The Bayesian engine is a mathematical algorithm that uses inferential logic to analyze historical data and shows the results as a predictive level as to the accuracy of a medical bill produced from the source documents. The inherent nature of Bayes like algorithms allows them to learn and improve their predictive capability through the use of a feedback system which is also part of the invention. Variations in algorithms and data flow can be easily made to support other predictive output related to billing or for the purposes of data mining and statistical evaluation.
