According to an aspect of the present invention, a medical device is provided, which comprises the following: (a) a hollow elongate body (e.g., a elongate cylinder, such as a needle) having distal and proximal ends; and (b) a rotatable member comprising a tissue morselizer and an elongate shaft (e.g., an auger-like tissue-drilling bit). In the devices of the present invention, the elongate shaft is disposed within the hollow elongate body and cooperates with the hollow elongate body to (i) advance material (e.g., morselated tissue) toward the proximal end of the hollow elongate body when the shaft is rotated in a first direction, and (ii) advance material (e.g., a therapeutic agent and/or a bulking agent) toward the distal end of the hollow elongate body when the shaft is rotated in a second direction that is opposite the first direction. According to another aspect of the invention a method of treatment is provided that comprises: (a) inserting the a medical device like that above into the tissue of a patient; (b) morselizing and removing tissue from within the patient while rotating the shaft in a first direction, thereby creating a void within the patient; and (c) introducing a therapeutic agent and/or a bulking agent into the void.

A system for performing a medical procedure and an introducer device suitable for use in the system. The system may be used, for example, to examine and/or to treat the uterus. According to one embodiment, the system includes an introducer, a morcellator, a flexible hysteroscope, and a fluid-containing syringe. The introducer is suitable for transcervical insertion into the uterus and includes a gun-shaped housing and a rigid sheath extending distally from the housing. The sheath is shaped to include an instrument lumen, a visualization lumen, and a pair of fluid lumens. The introducer also includes a first assembly within the housing for receiving the morcellator and for guiding the distal end of the morcellator into the instrument lumen and a second assembly within the housing for guiding the hysteroscope into the visualization channel. In addition, the introducer further includes an assembly for fluidly connecting the syringe to the fluid lumens.

Methods are disclosed, for performing therapeutic or diagnostic procedures at a remote site. According to one embodiment, the methods include the use of a system including an introducer designed for transcervical insertion into the uterus. The introducer is constructed to include a fluid lumen, an instrument lumen, and a visualization lumen. The system may include a fluid source, which is coupled to the fluid lumen and is used to deliver a fluid to the uterus either for washing the uterus or for fluid distension of the uterus. The system additionally includes a tissue modifying device, such as a morcellator, and a distension device for distending the uterus and/or for maintaining the uterus in a distended state. The tissue modifying device and the distension device are alternately deliverable to the uterus through the instrument lumen. The system may further include a hysteroscope deliverable to the uterus through the visualization lumen.

The invention relates to a tissue morcellator for minimally invasive surgery. The morcellator has a metallic cutting ring mounted on a visually transparent hollow cylinder, which in combination with a visually transparent outer morcellator tube enables a surgeon to visualize the inside of the morcellator shaft for detection of loose tissue fragments in the device. A tenaculum used with the morcellation device has a spacer for preventing contact with the blade. The cutting tube can oscillate, rather than rotate, along the longitudinal axis of the cutting tube. The morcellator utilizes an extendable tissue guide on the outer tube of the morcellator shaft for preventing the tissue from rotating along the longitudinal axis of the morcellator tube. This allows the tissue to be continuously rotated into the morcellator device for continuous peel. The tissue guide can also be fully retracted to allow for coring of the bulk tissue.

Embodiments of the invention provide a laparoscopic morcellating device and method for removing tissue from a body cavity. The morcellating device includes a containment mechanism having an aperture, a cutting mechanism designed to fit into an interior space of the containment mechanism and a retractor mechanism that is coupled to the cutting mechanism. The containment mechanism and cutting mechanism combination surrounds the tissue and the aperture of the containment mechanism is closed around the tissue. The morcellating device further includes a motor for actuating the retractor such that the cutting mechanism constricts and morcellates the tissue. The laparoscopic morcellating device further allows for torque balancing of the retractor mechanism, gas flow regulation of the body cavity, and safety feedback mechanisms that can alert the surgeon.

A surgical morcellator includes a trocar body portion and a handle portion detachably mounted to the trocar body portion. The trocar body portion is relatively lightweight and may be used separately from the handle portion with other laparoscopic instruments while morcellation is not required during the surgical procedure.

An improvement to a tissue removal device, that is, the addition of an adjustable protective guard and/or a retractable protective guard on the blade guard sheath of a mechanical tissue morcellator is presented. The retractable protective guard can be fixedly located at the distal end of a blade guard sheath and can be openly operated by a spring loaded means or a trigger means. The adjustable protective guard is moveably mounted in close contact around the blade sheath and may be positioned at any point along the length of the blade guard sheath in either direction. The addition of the adjustable protective guard and the retractable protective guard on the blade guard sheath of the mechanical tissue morcellator act to control and direct positioning of the tissue morcellator within the body cavity of the patient.

An endoscopic morcellator device and method for use is provided. The morcellator device includes an elongate shaft having a proximal end and a distal end and a lumen extending therethrough, a handle coupled to the proximal end of the shaft, at least one active electrode disposed about a periphery of the distal end of the shaft, at least one return electrode disposed at the distal end of the shaft and electrically insulated from the at least one active electrode, and at least one fluid conduit extending along the shaft and having an outlet at the distal end of the shaft.

A cutting blade for a morcellator has a generally cylindrical shape and includes a sidewall having an inner surface and an outer surface, each of which at least partially resides respectively in concentric first and second cylindrical planes. The blade sidewall further includes a beveled or sloped surface that extends from the inner surface toward the outer surface to define a sharpened edge situated in the second cylindrical plane in which the outer surface of the cutting blade sidewall resides. The sloped surface is provided to protect the sharpened edge of the morcellator blade when it is not obvious to a surgeon operating the morcellator that a tenaculum used to grasp anatomical tissue at the surgical site may have its tissue grasping claws spread to such an extent that the claws may contact the cutting blade of the morcellator. Under such circumstances, the tenaculum will contact the sloped portion of the inner surface of the cutting blade, rather than the sharpened edge, thereby minimizing the chance that the sharpened edge will become dull or damaged.

A morcellation system includes a morcellator and a grasper. The morcellator includes a housing and a tube extending from the housing and having at least one blade section. The morcellator further includes an oscillating mechanism configured to oscillate the tube. An end cap is coupled to the proximal end of the housing and is configured to receive the tube. The end cap defines a passage and includes an aligning structure disposed in the passage. The grasper includes a tubular member having a grasping end including first and second jaws moveable relative to one another. The grasper further includes a handle assembly operatively associated with the grasping end. A keyed collar is disposed about the tubular member. The keyed collar includes a key configured to cooperate with the aligning structure of the end cap to align the first and second jaws relative to the distal end of the tube.

The invention relates to a surgical instrument system encompassing a morcellator with a hollow shaft on whose distal end a cutting blade is configured, as well as a gripping instrument with a shaft on whose distal end a tool consisting of at least two jaw members and on whose proximal end a handle consisting of at least two gripping members is configured to activate the tool, in such a way that the gripping instrument can be inserted into the hollow shaft of the morcellator so that it can slide in the longitudinal direction of the morcellator shaft. To ensure versatile application along with simple structure, it is proposed with the invention that the morcellator with the insertable gripping instrument can be inserted into a hollow shaft of an additional medical instrument, in particular a resectoscope, and that an additional irrigation and/or suction canal is configured between the shaft of the morcellator and the inside of the hollow shaft of the additional medical instrument.

A morcellator shield and bag assembly including a shield having a cavity and a bag coupled to the shield. The bag is moveable between a contracted position, in which at least a portion of the bag is positioned within the cavity, and an expanded position. The bag has an opening that is in fluid communication with the cavity when the bag is in the expanded position. A method for using the assembly along with a morcellator to remove tissue from a body cavity. The method includes inserting at least a portion of the assembly into a body cavity, moving the bag to the expanded position, placing tissue in the bag, pulling the bag through an opening in a patient, inserting the morcellator into the assembly, and morcellating the tissue with the morcellator. The assembly is designed to contain the tissue being morcellated and prevent the morcellator from contacting the bag.

A morcellator includes a housing, an elongated tube extending distally from the housing, and first and second electrodes. A distal end of the elongated tube is configured to cut tissue. A minimally invasive laparoscopic tissue removal device, namely a morcellator. The device includes, from proximal end to distal end, a handle, handle guard, elongate sheath, and mouth, finally terminating in a safety tip. The elongate sheath and mouth are concentric, such that two elongate, rotating cutting/shredding blades/teeth and one elongate auger feed screw can be positioned along the length of the sheath and mouth. The teeth are opposingly disposed and staggered, such that contact with tissue mass grabs and pulls tissue inwards toward the auger, which transports the tissue proximally toward the outside of the patient's body. A motor unit and gear assembly can be coupled to the teeth and auger to drive rotation of the teeth and auger. A cup assembly can be disposed in overlying relation to the mouth for guiding the tissue mass towards the mouth. Various mechanisms of safely covering the mouth are contemplated as well.

Surgical instruments and surgical systems including the surgical instrument and a morcellator. The surgical instrument includes a tool assembly having an articulating joint and a screw positioned at a distal end of the articulating joint. The articulating joint is configured to articulate a distal portion of the tool assembly at an angle in relation to the longitudinal axis of the surgical instrument. The screw is configured to engage tissue, for example, a myoma in the uterine wall of a patient, and is configured to pitch and roll the tissue to expose cutting planes. A collapsible and expandable receptacle that can be inserted into a patient's cavity through a laparoscopic port (sleeve) is disclosed herein. The receptacle is configured so that when it enters the cavity it can be expanded or opened into a shape of a bowl or cone. A targeted tissue specimen can then be placed inside the expanded receptacle, and the surgeon has the room and visibility to cut the tissue with a power morcellator and avoid severing the bag or spreading unwanted particulate tissue to other areas inside the patient. Once the specimen has been cut and removed, the empty receptacle can be collapsed and removed out the same tissue removing sleeve.
