A device, kit and method may include or employ an implantable device (e.g., annuloplasty implant) and a tool operable to implant such. The implantable device is positionable in a cavity of a bodily organ (e.g., a heart) and operable to constrict a bodily orifice (e.g., a mitral valve). The tissue anchors may be guided into precise position by an intravascularly or percutaneously deployed anchor guide frame of the tool and embedded in an annulus of the orifice. Constriction of the orifice may be accomplished via a variety of structures, for example by cinching a flexible cable or via a anchored annuloplasty ring, the cable or ring attached to the tissue anchors. The annuloplasty ring may be delivered in an unanchored, generally elongated configuration, and implanted in an anchored generally arch, arcuate or annular configuration. Such may approximate the septal and lateral (clinically referred to as anterior and posterior) annulus of the mitral valve, to move the posterior leaflet anteriorly and the anterior leaflet posteriorly, thereby improving leaflet coaptation to eliminate mitral regurgitation.

Disclosed are implants and methods for remote remodeling of a mitral valve annulus. The implant comprises a body transformable from a flexible configuration for navigation to a treatment site, to a remodeling configuration for, in one application, applying pressure to the posterior leaflet of the mitral valve. On board electronics allow post deployment adjustment of the implant.

A valve implant or prosthesis includes a skirt or prosthetic valve leaflet configured to cover one of the leaflets of the valve to be repaired in a patient's heart. In one embodiment, a heart valve prosthesis includes a curved member and a skirt. The curved member can have first and second ends and be adapted to form a partial ring along a portion of one of the valve annulae in the patient's heart. Alternatively, the curved member can form a full ring that is adapted to extend along the entire valve annulus. The skirt extends along the curved member and depends therefrom. This prosthesis is especially useful in treating mitral valve insufficiency. In this case, the skirt can be configured so that when the prosthesis is secured to the mitral valve along the mitral valve annulus, the skirt covers the posterior leaflet and the opposed edges of the skirt and the anterior leaflet coapt. In addition, when the curved member is secured to the posterior portion of the mitral valve annulus, further annulus dilation can be minimized or eliminated. Implant delivery apparatus is provided for rapid implant delivery and securement to the valve.

A catheter based system for treating mitral valve regurgitation includes a restraining device having a flexible member, a plurality of movable anchor members attached to the outer surface of the flexible member, and an adjustment filament attached to the ends of the flexible member. One embodiment of the invention includes a method for attaching a flexible restraining device to the annulus of a mitral valve, and adjusting the length of the adjustment filament attached to the flexible member of the restraining device, thereby reshaping the mitral valve annulus so that the anterior and posterior leaflets of the mitral valve close during ventricular contraction.

A remodeling mitral annuloplasty ring with a reduced anterior-to-posterior dimension to restore coaptation between the mitral leaflets in mitral valve insufficiency (IMVI). The ring has a generally oval shaped body with a major axis perpendicular to a minor axis, both perpendicular to a blood flow axis. An anterior section lies between anteriolateral and posteriomedial trigones, while a posterior section defines the remaining ring body and is divided into P1, P2, and P3 segments corresponding to the three scallops of the same nomenclature in the posterior leaflet of the mitral valve. The anterior-to-posterior dimension of the ring body is reduced from conventional rings; such as by providing, in atrial plan view, a pulled-in P3 segment. Viewed another way, the convexity of the P3 segment is less pronounced than the convexity of the P1 segment. In addition, the ring body may have a downwardly deflected portion in the posterior section, preferably within the P2 and P3 segments. The downwardly deflected portion may have an apex which is the lowest elevation of the ring body and may be offset with respect to the center of the downwardly deflected portion toward the P1 segment. A sewing cuff may have an enlarged radial dimension of between 5-10 cm, or only a portion of the sewing cuff may be enlarged.

A method and apparatus for reducing mitral regurgitation. The apparatus is inserted into the coronary sinus of a patient in the vicinity of the posterior leaflet of the mitral valve, the apparatus being adapted to straighten the natural curvature of at least a portion of the coronary sinus in the vicinity of the posterior leaflet of the mitral valve, whereby to move the posterior annulus anteriorly and thereby improve leaflet coaptation and reduce mitral regurgitation.

Methods and apparatus for mitral valve repair are disclosed herein where the posterior mitral leaflet is supported or buttressed in a frozen or immobile position to facilitate the proper coaptation of the leaflets. An implantable apparatus may be advanced and positioned intravascularly beneath the posterior leaflet of the mitral valve. The apparatus may include one or more individual balloon members, each of which may be optionally configured with supporting integrated structures. A magnet chain catheter may be positioned within the coronary sinus and adjacent to the mitral valve to magnetically secure the apparatus in position beneath the posterior mitral leaflet. Alternatively, a split-ring device may be placed about the chordae tendineae supporting the mitral valve such that the ring slides along the chordae tendineae alternately against the mitral leaflet and towards the papillary muscles during systole and diastole.

Devices and methods for reshaping a mitral valve annulus are provided. One preferred device is configured for deployment in the right atrium and is shaped to apply a force along the atrial septum. A method and apparatus for treating mitral regurgitation by approximating the septal and lateral (clinically referred to as anterior and posterior) annulus of the mitral valve. The distal end of the device is inserted into the coronary sinus of the heart and the proximal end of the device rests within the right atrium along the tendon of Todaro and extends to at least the membranous septum of the tricuspid valve. Because the coronary sinus approximates the lateral (posterior) annulus of the mitral valve and the tendon of Todaro approximates the septal (anterior) annulus of the mitral valve, the device encircles approximately one half of the mitral valve annulus. The apparatus is then adapted to deform the underlying structures i.e. the septal annulus and lateral annulus of the mitral valve in order to move the posterior leaflet anteriorly and the anterior leaflet posteriorly and thereby improve leaflet coaptation and eliminate mitral regurgitation.

An improved prosthetic mitral valve is provided having advantageous hemodynamic performance, nonthrombogenicity, and durability. The valve includes a valve body having an inflow annulus and an outflow annulus. Commissural attachment locations are disposed adjacent the outflow annulus. An anterior leaflet and a posterior leaflet of the valve are shaped differently from one another. The inflow annulus preferably is scalloped so as to have a saddle-shaped periphery having a pair of relatively high portions separated by a pair of relatively low portions. The anterior high portion preferably is vertically higher than the posterior high portion.

A device, kit and method may include or employ an implantable device (e.g., annuloplasty implant) and a plurality of tissue anchors. The annuloplasty ring may be delivered in an unanchored, generally elongated configuration, and implanted in an anchored generally arched, arcuate or annular configuration. Such may approximate the septal and lateral (clinically referred to as anterior and posterior) annulus of the mitral valve, to move the posterior leaflet anteriorly and the anterior leaflet posteriorly, thereby improving leaflet coaptation to reduce mitral regurgitation.

The invention is an apparatus, system, and method for repairing heart valves. A suture line is secured to a papillary muscle, and then passed through a portion of a heart valve leaflet. A reference element is provided at a desired distance from a plane defined by the heart valve annulus. The suture line is secured to the heart valve leaflet at a position adjacent the reference element. The reference element may part of a device configured for placement on or in a heart valve annulus. The reference element may be slidingly secured to the device so that the distance of the reference element from the main body of the device can be varied by a surgeon or other user. The reference element may be a line of suture, which may be pre-installed during manufacture of the device or may be installed by the surgeon or other user.

Methods and apparatus are provided for valve repair. In one embodiment, the apparatus includes a first bridge portion and a second bridge portion. The apparatus may also include at least one base on each bridge portion. Attachment of the first bridge portion and the second bridge portion brings an anterior leaflet of the valve closer to the posterior leaflet and reduces a gap therebetween.

A system for treating mitral valve regurgitation includes a tensioning device having a flexible annuloplasty ring, a plurality of anchoring members and a tensioning filament attached to the flexible ring. One embodiment of the invention includes a method for attaching a flexible annuloplasty ring to the annulus of a mitral valve, and adjusting the lengths of segments of the tension filament attached to the flexible ring in order to exert force vectors on the annulus, thereby reshaping the mitral valve annulus so that the anterior and posterior leaflets of the mitral valve close completely during ventricular contraction.

Devices and methods for reshaping a mitral valve annulus are provided. One device according to the invention is configured for deployment in the right atrium and is shaped to apply a force along the atrial septum. In a further embodiment a device includes a middle anchor positioned on the bridge between the distal and proximal anchors.

Such a prosthesis implantable by catheterism includes mainly a docking station and a leaflet cooperating with the docking station. The leaflet is advantageously arranged in a configuration close to a posterior leaflet of a native mitral valve of a patient.

A system for treating mitral valve regurgitation includes a tensioning device having a plurality of helical anchors and a tensioning filament. One embodiment of the invention includes a method for attaching a tensioning device to the annulus of a mitral valve in a trans-leaflet configuration, and applying a tension force to the tension filament in order to exert force vectors on the annulus, thereby reshaping the mitral valve annulus so that the coaption of the anterior and posterior leaflets of the mitral is improved during ventricular contraction.
