A method and system for setting up and positioning a patient for radiotherapy without reference markers. Platform settings, including the parameters of the patient carrier platform, are determined. Platform location data is determined with the patient positioned on the platform for imaging without reference markers and includes 3D coordinates of the platform or the imaging coordinate system, calibrated to a predefined fixed set of initialization coordinates in the treatment room. Target tissue is imaged while the patient is on the platform. A treatment plan is generated based on the target tissue imaging. The treatment plan includes treatment fields and angles, encompassing platform positioning parameters. At the start of treatment, the patient is positioned on the platform without reference markers according to the platform settings.The platform is adjusted so that the patient is positioned in the treatment room in a supine position such that the target tissue is located in the isocenter of the treatment room according to the treatment plan, the platform location data and the coordinate system transformation of the imaging coordinate system into the treatment room coordinate system.

Embodiments herein relate to identifying a status of a patient such as in a dental or other medical treatment room. In one aspect, the solutions provide a system to determine whether the patient is anxious and / or a degree of anxiety of the patient. The solutions can include training a model such as a large language model (LLM). In a training phase, data is gathered from patients, the environments of treatment rooms and equipment of the treatment rooms during various medical procedures for a patient population. The model is then trained to correlate the data with a degree of anxiety. Once the model is trained, it can be deployed to determine whether an individual patient is anxious and to take appropriate countermeasures to reduce anxiety.
