An apparatus and method for assembling a soft tissue reconstruction system having first and second anchor assemblies. The apparatus includes a base plate, a first mounting block mounted to the base plate and having a first reference surface against which the first anchor assembly is mountable, a second mounting block slidably mounted to the base plate and having a second reference surface against which a bone anchor of the second anchor assembly is mountable, a measurement bar extending from the second mounting block and slidable relative to the first mounting block, a support block slidably mounted to the measurement bar having a third reference surface for abutting against a tissue presentation surface of the second anchor assembly, and a tensioning device for tensioning a graft attached to the first anchor assembly. Indicia on the measurement bar indicate the proper locations of the reference surfaces and components mounted/abutted thereto.

An adjustable, stand-alone tensioning system requires no additional fixturing, weights, or bone surface modification, and allows a single practitioner to provide an adjustable and repeatable tension to a soft tissue graft, and to install the final fixation implant. Its design facilitates introduction of the suture component of the graft into the tensioning process by simplifying retention of the suture. An even, regulated and reproducible tension is easily achieved without requiring the practitioner to manually pull on the suture strands to maintain graft tension.

Suture-based fasteners and fastening methods for soft tissue reconstruction are described that can include a needle tip swaged onto two sutures and loaded onto the end of a curved needle shaft. The needle shaft and tip can be driven through the tissues to be sutured and back out to where the needle tip can make a permanent connection with a locking bead. The needle shaft can be retracted, leaving two sutures extending from the side of the bead. One suture can be of nonabsorbable material and can be brought through the end of the locking bead opposite the tip and pulled to tighten the suture loop. The other suture can be of an absorbable material and can be used to rotate the locking bead under one of the layers of tissue being sutured after the suture loop is tightened. Following rotation, the second suture can be trimmed.

Disclosed herein are patient-specific craniofacial implants structured for filling bone voids in the cranium as well as for simultaneously providing soft tissue reconstruction and/or augmentation for improved aesthetic symmetry and appearance. Pterional voids or defects generally result from a chromic skull deformity along with a compromised temporalis muscle or soft tissue distortion from previous surgery. When muscle atrophy occurs in the pterion, temporal hollowing generally results where there would be soft tissue but for the atrophy. The patient-specific temporal implants herein are configured to have an augmented region adjacent the temporal region of the cranium in order to account for and correct any such temporal hollowing.

A device having one or more adjustable loops or coils associated with an implant body for use in soft tissue reconstructions is provided. One exemplary embodiment of a device includes a body and a suture filament, with the filament being used to form a self-locking sliding knot disposed on a top side of the body and a plurality of adjustable coils that are substantially disposed on the body's bottom side. Terminal ends of the filament located above the body's top side can be passed through an opening of a Lark's Head knot from opposite sides, thus forming a self-locking sliding knot, and then the terminal ends can be tensioned to adjust a circumference of the coils. Changing a coil's circumference changes a location of a ligament graft disposed on the coil. Other configurations of devices and systems, as well as methods for performing ACL repairs, are also provided.

Devices and methods for soft tissue reconstruction are provided. One exemplary embodiment of a device includes a body a suture filament extending through through-holes formed in the body. The suture is formed into an overhand knot having three collapsible openings with filament limbs extending from the knot. The limbs can be passed through the thru-holes of the body and selectively into the collapsible openings of the overhand knot to form coils extending to an opposite side of the body. For example, one limb can be passed through first and second collapsible openings to form two coils, while the other limb can be passed through the first and third collapsible openings to form two coils. The collapsible openings are collapsed around the limbs disposed in the openings, resulting in a secured overhand knot. Other configurations of knots, as well as devices and methods for use in tissue reconstruction, are also provided.

A device having an implantable body associated with at least two filaments or sutures and configured for use in soft tissue reconstructions is provided. One exemplary embodiment includes an implantable body 10 , an adjustable filament loop 42 for holding ligament grafts that is coupled to the body, and a shuttle suture 60 removably coupled to the implantable body and configured for shuttling the body through at least a portion of a bone tunnel. The loop can be defined by a self-locking knot, and one or more loop-adjusting limbs 52/54 can extend from the knot 46, with a portion of the limb(s) also extending into a hollow portion 66/68 of the shuttle suture. In some embodiments having two adjustable limbs, an intermediate portion of each limb can be the portions disposed in respective hollow portions of the shuttle suture. The present disclosure includes, in one embodiment, a tensioner instrument having a body having a channel and a plurality of arms projecting outward from the body, each arm including a cleat having a slot adapted to receive a suture; a base member positioned within the channel of the body; an actuating mechanism having a first coupling member adapted to engage a second coupling member on the body to removeably couple the actuating mechanism to the body; and a universal joint adapted to allow for relative movement between the base member and the body such that, when each suture is positioned within each cleat and the actuating mechanism is actuated, the tensioner applies uniform tension to each suture positioned in each cleat.

Described herein are systems and methods for soft tissue reconstruction and augmentation using a platelet-rich fibrin matrix. In one embodiment the system of the present invention includes a blood collection apparatus capable of drawing blood from a patient, a matrix preparation container configured to hold a patient's blood while the blood is separated and coagulated to form a platelet-rich fibrin matrix therein, and a matrix delivery device configured to receive the matrix preparation container therein and compress and force the platelet-rich fibrin matrix out of the matrix preparation container and into the patient. In another embodiment the invention described herein is a method in which blood is drawn from a patient and separated and coagulated to form a platelet-rich fibrin matrix. The platelet-rich fibrin matrix is implanted into a patient at the site of a tissue defect or where tissue augmentation is desired.

The apparatuses and methods described herein relates generally to the field of active agent (drug) release from surgical grafts useful for soft tissue reconstruction, regeneration, or repair. More particularly, described herein are surgical grafts for soft tissue repair that include active agent that is released over time while advantageously matching the biomechanical properties of tissue during healing and recovery.

A system for generating a reconstruction of an object of interest comprises a shape model generator (1) for generating a shape model representing a shape of the object in dependence on a plurality of projections of the object, and a reconstructor (2) for reconstructing the object, based on the projections, in dependence on the shape model to obtain the reconstruction of the object. The reconstructor (2) comprises a soft-tissue reconstructor (4) for generating a reconstruction favoring soft tissue, based on the plurality of projections, and a sparse reconstructor (5) for generating a reconstruction of sparse objects, based on the plurality of projections. The reconstructor (2) comprises a clipping subsystem (3) for clipping an outside of the object from the reconstruction, based on the shape model, or the reconstructor (2) is arranged for reconstructing only an inside and/or boundary of the object as defined by the shape model.

Disclosed is a bioresorbable implant with enhanced bone ingrowth and tissue integration utilizing an inside-out resorption mechanism and a method to manufacture a bioresorbable implants for use in osteotomies and bone-soft tissue reconstruction surgeries. The bioresorbable implant includes a polymer A (e.g., an aliphatic polymer matrix) and/or poly(propylene fumarate)), a carbohydrate B (e.g., a bioresorbable natural carbohydrate filler) and a ceramic C. The implant may be a porous scaffold structures with suitable porosity, pore size, pore interconnectivity, and mechanical properties for enhanced osteoblast penetration and bone formation to fabricate tissue integrating bioresorbable implants. The implant may be shaped as wedges, bone void fillers, and soft tissue fixation implant like screws, rods and/or anchors. In some embodiments, the implant may be a putty.
