A biocompatible tissue repair stimulating implant or “scaffold” device, and methods for making and using such a device, are provided. The implant includes one or more layers of a bioabsorbable polymeric foam having pores with an open cell pore structure. A reinforcement component is also present within the implant to contribute enhanced mechanical and handling properties. The implant houses a biological component that may be released to tissue adjacent the location in which the implant is implanted to faciliate and/or expedite the healing of tissue. This biological component resides primarily within the foam component of the implant, being incorporated within pores formed within the foam.

A method, system and apparatus for augmenting the surgical repair of soft tissue injuries, in which a first end of a bridge member attaches to a first portion of healthy tissue, and a second end of the bridge member attaches to a second portion of healthy tissue. The bridge member (or bridge members) used to augment the soft tissue repair may be interconnected or function independently. Flexibility and elasticity of the bridge member are determined by the situation and may be altered to improve healing. The device may be used in arthroscopic procedures, and may be manufactured in a variety of lengths, or may be manufactured one length and be cut to the desired length, or otherwise altered to provide an optimal length of the bridge member.

A system for repairing a meniscus includes a suture including a first anchor, a second anchor, and a flexible portion connecting the first anchor and the second anchor. The flexible portion includes a self-locking slide knot between the first anchor and the second anchor. The system also includes a needle having a longitudinal extending bore and an open end. The bore is configured to receive the first anchor and the second anchor. The system further includes a pusher configured to be movable within the bore of the needle. The pusher is configured to (1) discharge the first anchor and the second anchor, and (2) push the self-locking slide knot after the discharge of the second anchor.

A knotless suture anchor provides a method for securing soft tissue to a rigid structure such as bone. The anchor has an inner member that receives suture thread and rotates within an outer tubular member. As the inner member rotates, the suture thread is wrapped onto the inner member thereby increasing contact with the outer tubular member which applies pressure to the wrapped suture thread to retain it in position on the inner member while simultaneously and adjustably tensioning the suture thread. A ratchet prevents undesirable counter-rotation, although the ratchet can be released to permit loosening of the suture thread. Also described is a suture bridge and suture platform for providing further stabilization of a tendon. A method for repairing rotator cuff tears is also described.

An insertion tool is used to implant a knotless suture anchor to secure soft tissue to a rigid structure such as bone. The anchor has an inner member that receives suture thread and rotates within an outer tubular member. An inner shaft on the insertion tool engages the inner tubular member and is used to rotate the inner tubular member so that the suture thread is wrapped onto the inner tubular member thereby increasing contact with the outer tubular member of the anchor which applies pressure to the wrapped suture thread to retain it in position on the inner tubular member while simultaneously and adjustably tensioning the suture thread. The insertion tool has an outer shaft that engages the outer tubular member of the anchor and prevents the outer tubular member from rotating as the inner shaft on the insertion tool rotates the inner tubular member on the anchor.

Implant and delivery systems for soft tissue repair which affix soft tissue portions to a region of bone are described. Generally, the assembly includes two bone anchors preloaded through an inserter handle such that each anchor is penetrated into the bone directly. The first anchor is inserted through the tissue and into the bone, where it is locked into position. The first anchor has a suture or wire that protrudes through the tissue and threads through the second anchor and is secured to a rotatable suture reel located along the handle. The second anchor is inserted through the tissue and into the bone independently of the first anchor. Once the second anchor is deployed, the suture or wire is tensioned to secure the soft tissue to the bone and a suture plug within the second anchor is deployed to lock the suture or wire in place.

Stem cells products having the potential to support cells of a soft tissue lineage, and methods of preparation and use of those stem cell products are disclosed. The invention also relates to methods for the use of such stem cells products in the regeneration and repair of soft tissue, and in cell-based therapies for of soft tissue conditions.

The present invention provides hydrogels and compositions thereof for vocal cord repair or augmentation, as well as other soft tissue repair or augmentation (e.g., bladder neck augmentation, dermal fillers, breast implants, intervertebral disks, muscle-mass). The invention also provides methods of preparing and using the inventive hydrogels.

A composite implantable device for promoting tissue ingrowth therein comprising a biodurable reticulated elastomeric matrix having a three-dimensional porous structure having a continueous network of interconnected and intercommunicating open pores and a support structure is disclosed. The support structure may be a polymeric surgical mesh comprising a plurality of intersecting one-dimensional reinforcement elements, wherein said mesh is affixed to a face of said first matrix. Methods of making and using the implantable device are also provided.

Polyhydroxyalkanoate materials are provided which are suitable for repair of soft tissue, augmentation, and as viscosupplements in animals, particularly humans. The materials comprise liquid polyhydroxyalkanoate polymer compositions or polyhydroxyalkanoate microdispersions. Devices also are provided for storage and delivery of the polyhydroxyalkanoate compositions in vivo. Methods are provided for repairing or augmenting soft tissue in animals using the materials. In a preferred embodiment, the method include the steps of (a) selecting the animal soft tissue to be repaired or augmented; and (b) placing an injectable, liquid polyhydroxyalkanoate polymer or a polyhydroxyalkanoate microdispersion into the animal soft tissue, preferably using a minimally-invasive method such as injection. In another embodiment, the liquid polyhydroxyalkanoate polymer compositions or polyhydroxyalkanoate microdispersions are used as viscosupplements.

A soft tissue repair system comprising:

• • at least one anchor, the at least one anchor comprising a body, a bone-engaging element attached to the body for securing the body in bone, and a suture-attaching element attached to the body for attaching suture to the anchor, the suture-attaching element being configured so as to permit suture to be snared by the suture-attaching element after the anchor has been deployed in the bone.

The subject invention is a fixation device or anchor intended to secure a graft ligament or other soft tissue repair. The device is designed to be attached to a graft ligament and to secure the graft within a bone tunnel by extending transversely across the opening of the tunnel. In certain procedures the device is intended to pass through a bone tunnel in an orientation generally aligned with the tunnel and then turn transversely. The device has an inherent tendency to rotate upon exiting the tunnel to thereby become automatically transversely aligned with the tunnel and the graft ligament. The rotation is due to a moment automatically created as a resultant of forces applied to the anchor by a pulling-suture via a laterally offset aperture joining the pulling-suture to the anchor.

In a first embodiment, the present invention includes an instrumentation system for preparing a bone for soft tissue repair, the instrumentation system including a flexible drill pin capable of bending along a curved path; an aimer capable of engaging the flexible pin to bend the flexible pin; and a flexible reamer having a flexible portion along at least a portion of its length, the flexible portion comprising a plurality of laser cuts. In an alternate embodiment, the present invention may also include a method for preparing a bone tunnel in a femur adjacent a knee joint, the method including introducing a flexible drill pin into the knee joint; guiding the flexible drill pin towards a surface of the femur with an instrument introduced into the knee joint; drilling the flexible drill pin into the femur; removing the instrument from the knee joint; introducing a cannulated flexible reamer into the knee joint by placing the flexible pin within the cannulation of the flexible reamer; and reaming the bone tunnel in the femur along the path of the flexible pin.

Hydrogels intended for orthopedic applications, including repair and regeneration of cartilage, bone, joint surfaces and related tissues, must possess greater strength and toughness than hydrogels used in soft tissue repair. A hydrogel formulation is provided which has high strength, toughness, a suitable mechanical modulus and low equilibrium hydration. It may also have controlled porosity or degradation time. It can be made to polymerize in situ with high (“good” to “excellent”) adherence to target tissue or surfaces. A preferred formulation for forming such gels comprises 40 to 80% by weight of a low-molecular weight polar monomer and 30 to 10% of a hydrophilic macromeric crosslinker.

An implant and a process for preparing such an implant are disclosed. The implant includes a porous layer including collagen, a non-porous layer including a collagenic constituent, and a reinforcement component. The non-porous layer is joined to the porous layer and the reinforcement member is embedded into the non-porous layer. The porous layer has a three dimensional density ranging from about 20 mg collagen/cm³ to about 200 mg collagen/cm³.

Systems and methods for soft tissue to bone repairs, without knot tying. The soft tissue repair includes the steps of (i) securing first and second knotless fixation devices in bone, wherein the first fixation device is provided with a first loop and a first set of tails extending therefrom, and wherein the second fixation device is provided with a second loop and a second set of tails extending therefrom; (ii) passing one tail of the first fixation device through the second loop of the second fixation device; (iii) passing one tail of the second fixation device through the first loop of the first fixation device; and (iv) securing the first set of tails with a third fixation device and the second set of tails with a fourth fixation device.

An orthopaedic implant fixation using a surgically created bone cavity as a mold for forming an anchor from an in-situ hardenable material. An in-situ formed anchor of the present invention is especially useful for attaching an implant to osteoporotic cancellous bone. The injectable nature of the in-situ formed anchor allows implants to be adapted to minimally invasive surgical techniques. The present invention can be adapted to numerous implants or implant system components to include fasteners, pins, nails, intramedullary nails, and suture anchors. Applications include bone fracture fixation, bone fracture prevention, and soft-tissue repair.

The present invention provides a bionanocomposite including a pre-selected decellularized tissue crosslinked with a pre-selected nanomaterial. Also provided is a process for fabricating the bionanocomposite. Additionally, applications for using the bionanocomposite as soft tissue repair materials or scaffolds for tissue engineering are described.
