The surgical implant is introduced into at least one incision made on the upper wall of a vagina with the first soft tissue anchor inserted on a first side of the urethra behind the pubic bone, and the second soft tissue anchor inserted on a second side of the urethra behind the pubic bone, such that the suburethral support is suspended from the soft tissue anchors and supports the urethra. Each of the first and second soft tissue anchors are inserted in, and fix in, the soft tissue of the perineum without penetrating the obturator foramen.

Methods and Apparatus for Securing and Tensioning a Urethral Sling to Pubic Bone

Improved methods and apparatus to secure a urethral sling to pubic bone in a sub-urethral location to support the urethra and alleviate incontinence are disclosed. The urethral sling is preferably formed of a flexible sheet material extending between opposed first and second sling ends and bounded by opposed first and second sling sides. The urethral sling is selectively tensioned in a central portion intermediate the first and second sling ends and / or in one or both of end portions bracketing the central portion. In one embodiment, pre-positioned plicating lines in the central portion are selectively drawn and tied to slacken the central portion and tension the end portions. In further embodiments, bone anchor lines passed through locations displaced from the sling ends are drawn and fixed to tension the central portion between the displaced locations and slacken the end portions.

A device and method of using the device to assist in inserting a catheter in a female. The device includes a main body portion configured to be placed adjacent the pubic bone of a female, a pair of legs extending downwardly from the main body portion, a slot located between the legs, and a flange extending longitudinally along each of the legs. The flanges may extend along inner sides of the legs adjacent the slot. The device may also include a catheter guide formed at the upper end of the slot. The catheter guide may include a curved surface that extends along a portion of the main body member and each of the legs. Speculum

The opening area of the vagina contains most of its nerve endings; this area is generally much more sensitive to pressure than the inner two-thirds of the vagina. This portion of the vagina is also more constricted than the inner two-third of the vaginal area. When dilated, such as during a vaginal examination, pressure on and near the vaginal opening may increase pressure upon the adjacent urethral opening and the pubic bone, causing increased discomfort without providing additional expansion of the area. The middle area of the vagina is more dilatable, has fewer nerve endings, and more readily deforms when pressure is applied. The disclosed speculum is intended to place little pressure on the somewhat constricted vaginal opening and allow for increased expansion through the middle area, resulting in less discomfort to the patient during examination and maximized viewing area. A speculum and method of use are disclosed. Embodiments of speculum include upper and lower speculum blades. The upper blade may include a relatively wide paddle portion configured to provide a vaginal opening support surface when the speculum is in an open orientation. Another embodiment may include an upper speculum blade with a proximal portion located near the speculum handle which is stepped downward from a distal portion. The proximal portion preferably is configured to expand the vaginal opening less, while distal portion is configured to provide increased dilation or expansion to the vaginal middle area.
