Apparatus and methods for delivering fasteners during valve replacement

A fastener delivery tool includes a belt including pairs of features for releasably engaging tines of respective fasteners in a relaxed state defining a loop. The tool includes a loading chamber for receiving a fastener from the belt, a retaining member for limiting movement of the fastener within the loading chamber, an ejection track communicating with the loading chamber, a handle including an actuator, and a tongue and pusher member coupled to the actuator. Activation of the actuator advances the tongue to transform the fastener from the relaxed state to a U-shaped constrained state, and advances the pusher member to direct the fastener from the loading chamber down the ejection track. The tool may include a trigger for ejecting the fastener completely from the ejection track. The fastener may be used to secure a prosthetic heart valve or components thereof into surrounding tissue, e.g., within a tissue annulus.

The present invention, in the preferred embodiment, is a surgery recovery brassiere which resolves many of the problems faced by women who have undergone breast surgery and particularly, mastectomy. The present invention comprises front panels, prosthesis panels, rear panels, torso band, straps, drain tube apertures and fluid collection storage pouch.More specifically, two front panels each comprise a cup portion and a conforming portion. The prosthesis panels are attached to the rear facing of the cup portion of the front panel to form enclosed pockets to hold one or two prosthesis. Each of the two rear panels are joined at one end to the front panels and extend to meet the opposing rear panel at a back closure. The free end of each of the front panels meet to form a front closure. Straps extend from the top of the front panel to the top of the adjacent rear panel. The torso band extends around the lower edge of the surgery recovery brassiere to form an almost continuous strip of material along the lower edge of the front and rear panels.The surgery recovery brassiere further accommodates post-operative surgical drainage systems composed of one or more drain tubes and associated fluid collection bulbs. The position of drainage tube apertures, generally located proximate to the joint between the front and rear panels, allow drainage tubes to protrude from the chest wall, through the apertures and into the collection storage pouch. Tab closures allow the wearer to secure the drainage tubes within the aperture to further reduce movement of the tubes and disturbance of the drainage system. The storage pouch provides pockets with elastic top entry so that collection bulbs are within easy, convenient reach for emptying lymphatic fluids and other waste materials. The upper edge of the storage pouch releasably attaches to the torso band with a hook-and-loop fastener system.

A corner shelf assembly for corner walls using a flat shelf member with two perpendicularly disposed edges. Each of the edges includes at least one tongue member that extends coplanarly and is removably inserted within respective casing members that are cooperatively mounted within openings at the corner walls. The casing members accepting the tongues from one of the edges are wider so that the flat shelf member can be slid horizontally after this first set of tongues is inserted. A latching mechanism is used to keep the flat shelf member in place. Alternatively, corner the shelf assembly may include locking clips that are inserted in the casings after the user inserts the tongues in the wider casings. The locking clips prevent lateral movements of the shelf assembly. The corner shelf assembly can be removed upon the application of a predetermined amount of lifting force to the shelf member.

A tongue retaining system is disclosed herein generally having, in one variation, a two-part system. A covering can be sized to fit at least partially over or upon a tongue of a patient, where the covering has an engagement mechanism positioned thereon. A retaining device, sized to fit within a mouth of the patient, may be positioned upon the teeth and have a corresponding engagement mechanism positioned thereon for temporarily coupling with the engagement mechanism on the covering. Such a system can be used to maintain or hold a natural position of the tongue within the mouth of the patient to maintain patency of the airway passages.

The lower blade of the invention has a distal end adapted as an esophageal obturator for obstructing the esophagus. An arytenoid bumper to prevent the laryngoscope from penetrating too far into the esophagus is preferably secured to the distal end of the lower blade. The arytenoid bumper may have a ramp or wedge shape adapted to deflect an intubation tube through a patient's larynx and into the trachea. The upper blade optionally has a tongue guard adapted to hold the tongue outside of a channel formed by the lower blade, the tongue guard and the upper blade. A suction conduit is optionally built in or secured to either the lower blade or the upper blade for providing a tube for suctioning secretions or emesis. A suction break is located in the handle of the laryngoscope to allow a finger of the hand holding the laryngoscope to control the presence and intensity of suction.
