Method for preparing two-layer bicomposite collagen material for preventing post-operative adhesions

A bicomposite material based on collagen is prepared which has two closely bound layers and is biocompatible, non-toxic, hemostatic and biodegradable in less than a month, and can be used in surgery to achieve hemostasis and prevent post-surgical adhesion. To prepare the material, a solution of collagen or gelatin, which may contain glycerine and a hydrophilic additive such as polyethylene glycol or a polysaccharide, is poured onto an inert support to form a layer 30 .mu.m to less than 100 .mu.m thick. Then a polymeric porous fibrous layer is applied during gelling of the collagen or gelatin, and the resultant material is dried. The polymeric porous fibrous layer may be made of collagen or a polysaccharide, and have a density of not more than 75 mg / cm.sup.2, a pore size from 30 .mu.m to 300 .mu.m and a thickness of 0.2 cm to 1.5 cm.

Methods and devices for improving percutaneous access in minimally invasive surgeries

A device for use as a portal in percutaneous minimally invasive surgery performed within a patient's body cavity includes a first elongated hollow tube having a length adjusted with a self-contained mechanism. The first elongated tube includes an inner hollow tube and an outer hollow tube and the inner tube is adapted to slide within the outer tube thereby providing the self-contained length adjusting mechanism. This length-adjustment feature is advantageous for percutaneous access surgery in any body cavity. Two or more elongated tubes with adjustable lengths can be placed into two or more adjacent body cavities, respectively. Paths are opened within the tissue areas between the two or more body cavities, and are used to transfer devices and tools between the adjacent body cavities. This system of two or more elongated tubes with adjustable lengths is particularly advantageous in percutaneous minimally invasive spinal surgeries, and provides the benefits of minimizing long incisions, recovery time and post-operative complications.

Biocompatible crosslinked polymers, and methods for their preparation and use, are disclosed in which the biocompatible crosslinked polymers are formed from water soluble precursors having electrophilic and nucleophilic functional groups capable of reacting and crosslinking in situ. Methods for making the resulting biocompatible crosslinked polymers biodegradable or not are provided, as are methods for controlling the rate of degradation. The crosslinking reactions may be carried out in situ on organs or tissues or outside the body. The invention disclosed herein is a tissue-anchoring system, including a tissue-anchoring device and tissue anchors. The tissue-anchoring device includes a housing and a tissue anchor positioned on the tissue-anchoring device that will be inserted into a tissue or secured onto a tissue. The device may optionally have a plunger assembly slidably positioned in the housing to assist advancing anchor into a tissue. The tissue anchor has a barb end and a shaft. Preferred embodiments include surgical meshes coated with one or more biodegradable polymers that can act as a stiffening agent by coating the filaments or fibers of the mesh to temporarily immobilize the contact points of those filaments or fibers and / or by increasing the stiffness of the mesh by at least 1.1 times its original stiffness. The devices of the invention can also provide relief from various post-operative complications associated with their implantation, insertion or surgical use. Translational bone fixation assemblies, kits containing such assemblies, and methods of use are described herein. The described assemblies may be used in spinal fusion procedures in which a damaged or diseased disc (or part of a disc) is removed from between a pair of vertebrae and a spinal fusion spacer is placed between the vertebrae. The assemblies may be applied to an anterior portion of the affected vertebrae to span the affected disc space, and may be fixed to the vertebrae using bone screws. The assemblies may function to maintain the vertebrae aligned during the initial period following fixation in which fusion of the spacer to the adjacent vertebrae occurs. The assemblies may also function to share some of the axial spinal load applied to the fusion spacer to prevent extreme subsidence of the spacer into the vertebral body, such as where the patient has poor bone quality. The assemblies may also act to prevent the spacer from being expelled from the disc space during the initial post-operative period. Finally, these assemblies may be used in other parts of the body where the advantages of the assemblies are desired or applicable.

A surgical sponge comprises a plurality of radiopaque markers having a high radiographic density and a distinctive, visually recognizable shape. The markers have an x-ray density equivalent to at least about 0.1 g / cm2 of BaSO4. The markers produce an x-ray image with high contrast and a shape that is readily recognizable and differentiated from the images produced by other items and structures commonly seen in x-rays of post-operative patients. Owing to the distinctive, high contrast image produced by the markers, the sponge is reliably and unambiguously detected. This is so even in situations where the sponge is inadvertently left in the surgical wound. Discomfort, trauma, and possibly fatal consequences that might otherwise occur are virtually eliminated. A method of treating glaucoma in an eye by managing fluid flow past an implanted shunt having an elastomeric plate and a non-valved elastomeric drainage tube. The plate is positioned over a sclera of the eye with an outflow end of the elastomeric drainage tube open to an outer face of the plate. An inflow end of the drainage tube tunnels through the sclera to the anterior chamber of the eye. The plate may have regions of greater propensity for cell adhesion alternating with regions of lesser cell adhesion. For example, regions of texturing around the plate or drainage tube may be provided to control the size of a bleb that forms over the implant. The effective surface area of the plate may be balanced against a number of fenestrations. The drainage tube has a reduced profile and may be shaped with a non-circular external cross-section to reduce its height. A scleral groove may be used to further reduce the height of the drainage tube on the sclera. A flow restrictor for the early post operative period will immediately lower the intraocular pressure (IOP) and simultanously prevent hypotony.

The invention provides implants suitable for use as an artificial cornea, and methods for making and using such implants. Artificial corneas having features of the invention may be two-phase artificial corneas, or may be three phase artificial corneas. These artificial corneas have a flexible, optically clear central core and a hydrophilic, porous skirt, both of which are biocompatible and allow for tissue integration. A three-phase artificial cornea will further have an interface region between the core and skirt. The artificial corneas have a high degree of ocular tolerance, and allow for tissue integration into the skirt and for epithelial cell growth over the surface of the prosthesis. The use of biocompatible material avoids the risk of disease transmission inherent with corneal transplants, and acts to minimize post-operative inflammation and so to reduce the chance or severity of tissue necrosis following implantation of the synthetic cornea onto a host eye.

The present invention comprises front panels, prosthesis panels, rear panels, torso band, straps, drain tube apertures and fluid collection storage pouch.More specifically, two front panels each comprise a cup portion and a conforming portion. The prosthesis panels are attached to the rear facing of the cup portion of the front panel to form enclosed pockets to hold one or two prosthesis. Each of the two rear panels are joined at one end to the front panels and extend to meet the opposing rear panel at a back closure. The free end of each of the front panels meet to form a front closure. Straps extend from the top of the front panel to the top of the adjacent rear panel. The torso band extends around the lower edge of the surgery recovery brassiere to form an almost continuous strip of material along the lower edge of the front and rear panels.The surgery recovery brassiere further accommodates post-operative surgical drainage systems composed of one or more drain tubes and associated fluid collection bulbs. The position of drainage tube apertures, generally located proximate to the joint between the front and rear panels, allow drainage tubes to protrude from the chest wall, through the apertures and into the collection storage pouch. Tab closures allow the wearer to secure the drainage tubes within the aperture to further reduce movement of the tubes and disturbance of the drainage system.
