External pulse generator for adjunct (add-on) treatment of obesity, eating disorders, neurological, neuropsychiatric, and urological disorders

An external pulse generator comprising a primary coil and adapted to inductively couple to an implanted receiving means, is designed to deliver neuromodulation therapy for disorders comprising obesity, eating disorders, anxiety and the like. The external pulse generator contains limited number of predetermined programs. These programs provide the patient or caretaker a means to adjust the therapy within confined limits, or turn the device off. The predetermined programs contain unique combination of pulse amplitude, pulse width, frequency of stimulation, and on-off time. In another mode of operation, the parameters can be individually adjusted and the stimulation therapy program can be “customized” for the patient, and stored in the memory. The programs are capable of being modified with a programming station connected to the pulse generator with a RS232-C serial connection. Additionally, the external pulse generator has two-way wireless capabilities, whereby enabling the physicians to remotely control the therapy programs of their patients using wireless internet. Moreover, the external pulse generator has proximity sensing and feedback regulation component to provide a constant therapy in accordance with the predetermined programs.

Method for monitoring impedance to control power and apparatus utilizing same

A method for monitoring the impedance in a circuit coupling a radio frequency electrode to a radio frequency generator to control the power supplied by the generator to the electrode during a treatment procedure. In the method, the impedance in the circuit is monitored over a length of time. An expected impedance at the end of the treatment procedure is calculated from the monitored impedance and compared to a predetermined maximum impedance. The power supplied to the circuit is reduced if the expected impedance is greater than the predetermined maximum impedance. A computer-readable memory and apparatus utilizing the method are provided.

Implantable medical device eluting drug locally and in prolonged period is provided, including several types of such a device, the treatment modes of implementation and methods of implantation. The device comprising of polymeric substrate, such as a matrix for example, that is used as the device body, and drugs, and in some cases additional scaffolding materials, such as metals or additional polymers, and materials to enhance visibility and imaging. The selection of drug is based on the advantageous of releasing drug locally and in prolonged period, where drug is released directly to the extracellular matrix (ECM) of the diseased area such as tumor, inflammation, degeneration or for symptomatic objectives, or to injured smooth muscle cells, or for prevention. One kind of drug is the gene silencing drugs based on RNA interference (RNAi), including but not limited to si RNA, sh RNA, or antisense RNA / DNA, ribozyme and nucleoside analogs. The modes of implantation in some embodiments are existing implantation procedures that are developed and used today for other treatments, including brachytherapy and needle biopsy. Imaging based symptomatic classification and cardiovascular stroke risk score estimation

Characterization of carotid atherosclerosis and classification of plaque into symptomatic or asymptomatic along with the risk score estimation are key steps necessary for allowing the vascular surgeons to decide if the patient has to definitely undergo risky treatment procedures that are needed to unblock the stenosis. This application describes a statistical (a) Computer Aided Diagnostic (CAD) technique for symptomatic versus asymptomatic plaque automated classification of carotid ultrasound images and (b) presents a cardiovascular stroke risk score computation. We demonstrate this for longitudinal Ultrasound, CT, MR modalities and extendable to 3D carotid Ultrasound. The on-line system consists of Atherosclerotic Wall Region estimation using AtheroEdge™ for longitudinal Ultrasound or Athero-CTView™ for CT or Athero-MRView from MR. This greyscale Wall Region is then fed to a feature extraction processor which computes: (a) Higher Order Spectra; (b) Discrete Wavelet Transform (DWT); (c) Texture and (d) Wall Variability. The output of the Feature Processor is fed to the Classifier which is trained off-line from the Database of similar Atherosclerotic Wall Region images. The off-line Classifier is trained from the significant features from (a) Higher Order Spectra; (b) Discrete Wavelet Transform (DWT); (c) Texture and (d) Wall Variability, selected using t-test. Symptomatic ground truth information about the training patients is drawn from cross modality imaging such as CT or MR or 3D ultrasound in the form of 0 or 1. Support Vector Machine (SVM) supervised classifier of varying kernel functions is used off-line for training. The Atheromatic™ system is also demonstrated for Radial Basis Probabilistic Neural Network (RBPNN), or Nearest Neighbor (KNN) classifier or Decision Trees (DT) Classifier for symptomatic versus asymptomatic plaque automated classification. The obtained training parameters are then used to evaluate the test set. The system also yields the cardiovascular stroke risk score value on the basis of the four set of wall features.

Occlusion device for occluding an atrial auricula and method for producing same

A self-expanding occlusion device for occluding an atrial auricula in the heart of a patient includes a braiding of thin wires or threads given a suitable profile form by means of a molding and heat treatment procedure. The occlusion device includes a proximal retention area, a distal retention area and a center section. The occlusion device holds securely in the atrial auricula of a patient in its expanded and implanted state without damaging the tissue of the patient's heart. The proximal retention area has a flanged area which positions at the inner walls of the atrial auricula when the occlusion device is in an expanded state in the atrial auricula to be occluded, and forms a force-fit connection with the inner walls of the atrial auricula, thus holding the implanted and expanded occlusion device in the atrial auricula, whereby the distal retention area closes the opening in the atrial auricula.

The present invention provides a method for exchanging catheters while containing emboli within a blood vessel such as a saphenous vein graft, coronary artery, carotid artery, or other similar vessels. A guidewire is inserted through the vasculature of a patient until it reaches a desired treatment site. A therapy catheter is then inserted over the guidewire until the distal end of the therapy catheter reaches the treatment site. The guidewire has a distally mounted balloon which is inflated to occlude the blood vessel. Then, the therapy catheter provides means for treating the vessel at the treatment site. After treatment, the therapy catheter is removed from the guidewire and exchanged with an aspiration catheter which rides over the guidewire until the distal end of the aspiration catheter reaches the treatment site. The aspiration catheter applies negative pressure to remove any emboli formed by the treatment procedure.
