Valved Hemodialysis Catheter

A dialysis catheter includes an elongate catheter member having an outer wall member defining proximal and distal end regions and a longitudinal axis, and defining first and second longitudinal lumens. The outer member includes first and second ports in respective fluid communication with the first and second longitudinal lumens. The first opening and the first lumen define an inflow passage for removal of blood under vacuum for circulation within a hemodialysis machine. The second opening and the second longitudinal lumen define an outflow passage for return of the blood from the hemodialysis machine. A valve is disposed relative to the outer member proximal of the first port. The valve is adapted to move from a closed position to an open position in response to a predetermined vacuum pressure level realized within the first longitudinal lumen to permit flow of blood through the valve and into the first longitudinal lumen. The valve may be adapted to move to the open position in the event of an at least partial luminal occlusion within the first longitudinal lumen at locations distal of the valve.

A convertible multi-lumen catheter that may be used for hemodialysis or other indications involving infusion and / or withdrawal of fluids from the body. Unlike existing catheters having a set number of lumens which may limit their utility as both short and long-term venous vascular devices, the catheter of the invention allows one or more additional lumens during the acute phase of catheter use with removal of these lumens (i.e. conversion) for more permanent use. A typical example of this would be a triple lumen device for hemodialysis and antibiotic therapy during an acute infection with conversion to a chronic dual lumen hemodialysis catheter after successful treatment of the infection. The lumen is permanently or semi-permanently blocked using a biocompatible plastic obturator that is inserted into the unused lumen and locked and / or glued into place.

A catheter apparatus is provided for reducing blood clotting in connection with hemodialysis treatment. The catheter apparatus can comprise a first conduit defining an arterial lumen and a second conduit defining a venous lumen. Large staggered apertures can be provided in side walls of the conduits with at least one of the apertures having a cross-sectional area equal or greater than a cross-sectional area of one of the lumens. The catheter apparatus can further include first and second removable obturators adapted for axial insertion into the conduits and for occluding the apertures while so inserted. Advantageously, the obturators can wipe the apertures during removal, causing blood clots incident on the apertures to be dislodged from the apertures. Related methods for using the catheter apparatus in performing hemodialysis treatment are also provided.

A hemodialysis catheter assembly with a flexible catheter lead that can control blood flow at a hemodialysis access site even under high blood pressures encountered at such sites. Flexible tubing extends through the hollow interior of a housing having opposite, open ends. The assembly includes components to alternately occlude or not occlude the lumen of the flexible tubing. In a first embodiment, spring clips press against the tubing, but insertion of a hollow sleeve into the lumen thereof retracts the clips and provides a flow path. In a second embodiment, a pinch bar can be manually moved into and out of engagement with the flexible tubing to occlude or open a flow path. In a third embodiment, the housing has a screw cap. A spring-loaded pinch bar presses against, and occludes the lumen of, the collapsible tubing when the screw cap is screwed into a threaded cutout in the housing, and provides a flow path through the tubing when the screw cap is removed. In a fourth embodiment, the tang portion of a tongue when in an extended position presses against, and occludes the lumen of, the compressible tubing; advancement of a capture ring attached to a sliding button retracts the tang portion and opens a flow path.
