Portable remote patient telemonitoring system

A system and method for monitoring vital signs and capturing data from a patient remotely using radiotelemetry techniques. The system is characterized by a cordless, disposable sensor band with sensors form measuring full waveform ECG, full waveform respiration, skin temperature, and motion, and transmission circuitry for the detection and transmission of vital signs data of the patient. A small signal transfer unit that can either be worn by the patient, e.g., on his or her belt, or positioned nearby receives data from the sensor band, which it then forwards by e.g., radio transmission to a base station that can be located up to 60 meters away. The base station receives data transmissions from the signal transfer unit and is designed to connect to conventional phone lines for transferring the collected data to a remote monitoring station. The base station may also capture additional clinical data, such as blood pressure data, and to perform data checks. Patient safety is enhanced by the ability of the base station to compare clinical data, e.g., ECG, against given profiles and to mark events when appropriate or when the base station is programmed to do so. Such events are indicated to the physician and could be indicated to the patient by reverse transmission to the signal transfer unit. A remote monitoring station allows the presentation and review of data (including events) forwarded by the sensor band. ECG analysis software and a user-friendly graphical user interface are provided to remotely analyze the transmitted data and to permit system maintenance and upkeep. The system of the invention has useful application to the collection of patient clinical data during drug trials and medical testing for regulatory approvals as well as management of patients with chronic diseases.

An interactive computer assisted method reviews, and analyzes, a patient who needs one or more medications using a computer and a user associated therewith. The method includes the steps of pre-selecting patients to obtain a preliminary set of patients eligible for the interactive computer assisted method responsive to first predetermined criteria, and filtering the preliminary set of patients to identify and form a secondary set of patients from the preliminary set of patients having a greater likelihood of benefiting from the interactive computer assisted method responsive to second predetermined criteria. The method also includes the steps of enrolling a patient from the secondary set of patients, and communicating with the patient to obtain information to assist the user in determining whether therapy and/or medication issues are relevant. A method useful for facilitating choosing a treatment or treatment regime and for predicting the outcome of a treatment for a disorder which is diagnosed and monitored by a physician or other appropriately trained and licensed professional, such as for example, a psychologist, based upon the symptoms experienced by a patient. Unipolar depression is an example of such a disorder, however the model may find use with other disorders and conditions wherein the patient response to treatment is variable. In the preferred embodiment, the method for predicting patient response includes the steps of performing at least one measurement of a symptom on a patient and measuring that symptom so as to derive a baseline patient profile, such as for example, determining the symptom profile with time; defining a set of a plurality of predictor variables which define the data of the baseline patient profile, wherein the set of predictor variables includes predictive symptoms and a set of treatment options; deriving a model that represents the relationship between patient response and the set of predictor variables; and utilizing the model to predict the response of said patient to a treatment. A neural net architecture is utilized to define a non-linear, second order model which is utilized to analyze the patient data and generate the predictive database from entered patient data.

System and method for automatic generation of search results based on local intention

A system and related techniques automatically analyze Web search and other activity, to generate locality-selected results based on not just the user's location, but also the business or other content provider's location and the degree of local intent in the user's query. The locality or region to the user may be identified, for instance, by the presence of geographic clues or indicators in the general content of the Web site, for instance, the presence of ZIP codes, telephone numbers, town names or other semantic or other indicators which have some geographic connotation in search terms, key words or other query or semantic inputs. The user's degree of local intent may be automatically analyzed as well, for instance by the presence of terms such as “car repair” or “pizza restaurant” which may suggest the user intends to locate local goods, services or providers. The search service may then access a content database to identify ads or other media or content which match or correspond to the detected locality or region, such as ads for local restaurants, car dealerships, physicians or other services or products, and which correspond in location, working radius and degree of localness to the user's query. Because the delivery of localized search results according to the invention is specifically filtered for user-driven local intent, advertisements and other media or content may be delivered which better match the user's search objectives.

Everted filter device

Everting filter devices and methods for using the devices, including using the devices as intra-vascular filters to filter thrombus, emboli, and plaque fragments from blood vessels. The filter devices include a filter body nominally tubular in shape and having a large proximal opening. The filter body can extend from a proximal first end region distally over the non-everted exterior surface of the filter, further extending distally to a distal-most region, then converging inwardly and extending proximally toward the filter second end region, forming a distal everted cavity. The degree of eversion of the filter can be controlled by varying the distance between the filter first end region near the proximal opening and the closed second end region. Bringing the filter first and second end regions closer together can bring filter material previously on the non-everted filter exterior to occupy the distal-most region. The everting process can also bring filter material previously in the distal-most position further into the distal everted cavity. The filter devices can be used to remove filtrate from body vessels, with the filtrate eventually occluding the distal-most region. The filter can then be further everted, bringing fresh, unoccluded filter material into place to provide additional filter capacity. Some everting filters have the capability of switching between occluding and filtering modes of operation, thereby allowing a treating physician to postpone the decision to use filtering or occluding devices until well after insertion of the device into the patient's body.

System and method for recording patient history data about on-going physician care procedures

A system and method for processing patient data permits physicians and other medical staff personnel to record, accurately and precisely, historical patient care information. An objective measure of a physician's rendered level of care, as described by a clinical status code, is automatically generated. Data elements used in the determination of the generated clinical status code include a level of history of the patient, a level of examination of the patient, a decision-making process of the physician treating the patient, and a "time influence factor." The quantity and quality of care information for a particular patient is enhanced allowing future care decisions for that patient to be based on a more complete medical history. Enhanced care information can be used in outcome studies to track the efficacy of specific treatment protocols. Archiving of patient information is done in a manner which allows reconstruction of the qualitative aspects of provided medical services. The medical care data can be recorded, saved, and transferred from a portable system to a larger stationary information or database system. Considerable physician and staff time are saved and precision and accuracy are significantly enhanced, by generating these clinical status codes automatically (at the point of service by the care-provider without any intermediary steps) from information recorded simultaneously with the provision of services.
