Evidence-based medicine (EBM) is an approach to medical practice intended to optimize decision-making by emphasizing the use of evidence from well-designed and well-conducted research. Although all medicine based on science has some degree of empirical support, EBM goes further, classifying evidence by its epistemologic strength and requiring that only the strongest types (coming from meta-analyses, systematic reviews, and randomized controlled trials) can yield strong recommendations; weaker types (such as from case-control studies) can yield only weak recommendations. The term was originally used to describe an approach to teaching the practice of medicine and improving decisions by individual physicians about individual patients. Use of the term rapidly expanded to include a previously described approach that emphasized the use of evidence in the design of guidelines and policies that apply to groups of patients and populations ("evidence-based practice policies"). It has subsequently spread to describe an approach to decision-making that is used at virtually every level of health care as well as other fields (evidence-based practice).

A system and method for automatically integrating real-time, ad-hoc, community generated commentary such as comments, notes, observations, and anecdotal data in any human perceptible form with medical treatment guidelines (otherwise known as “clinical guidelines”, “clinical protocols”, or “clinical practice guidelines”) to create a display that shows the medical treatment guideline alongside the commentary. The present invention employs a server with a database capable of receiving and displaying commentary from local or remote users via a network, such as the Internet using various terminal devices such as a computers, smartphones, or tablets. The present invention provides an accessible, dynamically revised stream of commentary thus creating an evolving resource in which infrequently revised medical treatment guidelines are supplemented with commentary in real-time. The utilization of medical treatment guidelines by health-care providers now characterized as “evidence based medicine” or “evidence based practice” is thus advanced constituting what is termed as “collaborative evidence-based practice.”

Method and apparatus to support evidence based medicine
A method and apparatus for providing reports of medical procedures includes a biometric data recorder to record and transmit biometric data of a patient, the biometric data being transmitted with a medical report of the medical procedure. The medical report and biometric data are transmitted as an encrypted transmission to an information center for storage. The medical reports of steps in the medical procedure for a patient are linked using the biometric data even if performed by different medical service providers. Medical reports of plural patients undergoing the procedure are stored, linked according to patient using the patient biometric data. Reports generated from the linked data anonymously report a given patient's status following the procedure. Statistical reports are generated on plural patients undergoing the procedure, and competing procedures are compared using the statistical reports.
