Medical devices, systems, and methods reduce the distance between two points in tissue, often for treatment of congestive heart failure and often in a minimally invasive manner. An anchor is inserted along an insertion path through a first wall of the heart. An arm of the anchor is deployed and rotationally positioned according to a desired alignment. Application of tension to the anchor may draw the first and second walls of the heart into contact along a desired contour so as to effect a desired change in the geometry of the heart. A method for performing radiological-image-guided percutaneous surgery with a system which includes a radiological image generating device for generating an image of a target anatomy of a patient, and a needle insertion mechanism disposed adjacent the image generating device and having a needle adapted to be inserted into the patient. The method includes the steps of: determining a needle trajectory of the needle by positioning the image generating device for aligning, in the image generated by the image generating device, a desired skin insertion site of the patient with a target region of the target anatomy; locking the needle in a direction of the needle trajectory; and repositioning the image generating device to obtain a lateral view of the needle trajectory for viewing an insertion depth and path of the needle during its insertion into the patient. An apparatus for bracing a plurality of vertebrae of the human spine has at least two pedicle screws, each having an annular head with an opening therewithin and including a slot therewithin. Each slot extends into the corresponding opening and includes internal threaded portions. A securing screw to be screwed in each slot is provided. An insertion device is used to deform a deformable intraocular lens to a reduced size and insert the deformed lens into an eye through an insertion tube. An instrument for correction of spinal deformities includes an inner member with a proximal portion configured to receive a corrective force and a distal coupling portion configured to transfer the corrective force to a spinal implant in a vertebra. A slot extends transversely through the coupling portion and is configured to guide a fixation rod into the spinal implant.
