An expansile polymeric material, such as a swellable polymer (e.g., a hydrogel), a flexible or elastomeric polymer foam (e.g. silicone, polyurethane, etc.) or a carrier member (e.g, a coil, filament, wire, etc) that carries a quantity of such expansile polymer is delivered into a perigraft space (i.e., space between the endovascular graft and the surrounding blood vessel wall) such that the polymeric material expands in situ to substantially fill the perigraft space or a portion thereof. The expansile polymeric material is delivered into he perigraft space through a catheter and/or cannula that is placed prior to, during or after the implantation of the endovascular graft. The invention includes an injector apparatus that is useable to deliver the expansile polymeric material through the wall of a previously implanted graft. After delivery into the perigraft space, the expanded polymeric material expands so as to fill all or an intended portion of the perigraft space in a manner that substantially prevents additional blood from leaking or flowing into such perigraft space. One type of blood-absorbing, porous, expansile polymeric material useable in this invention is a super-expansile hydrogel.

The invention relates to a surgical instrument for at least partially occluding a body conduit comprising a first jaw, a second jaw opposing the first jaw and moveable relative to the first jaw to occlude the conduit, and an insert being mounted by at least one of the first and second jaws with a length of the insert extending along at least one of the first and second jaws. One of the first and second jaws may further comprise a retention barb to provide a friction fit with the insert. The insert is flexible, disposable and may be formed by injection molding. The retention barb may be a three-sided or multiple-sided barb that is formed at the proximal end of at least one of the first and second jaws. The first and / or second jaws may be bent and / or curved. In another aspect, the insert comprises an outer wall and an inner wall for fitting over the jaw, and the inner wall further comprises a molded, rectangular raised ring formed at the proximal end to aid the retention of the insert over the retention barb of the jaw. The insert may be removed from the jaw by pulling the insert in a distal direction to overcome the friction fit of the retention barb. The insert may further include a soft tip covering the distal end of one of the jaws to minimize trauma during surgery. The distal ends of the jaw and the insert may be tapered to allow ease of installation and removal. In another aspect, the insert and jaws may be removably attached to one another in a snap-fit relationship where the jaws may include one or more holes and the insert may include one or more projections. It is appreciated that the holes and projections may vary in width, depth and height depending on application.

After implantation of a reservoir of the first embodiment in an organism, cultured cells obtain food and oxygen via diffusion in tissue fluid through the porous walls of the interior chamber, while metabolic products, including drugs, diffuse away from the cell culture in an analogous manner. In a reservoir of the second embodiment, a biodegradable matrix substantially fills the pores (voids), and progressive dissolution of the matrix releases one or more drugs into surrounding tissue fluid. Reservoirs of either embodiment comprise a plurality of voids of a predetermined size effective for stimulating angiogenesis from the surrounding vascular tissue into at least a portion of the reservoir. The reservoir thus acts to couple a source: of drugs to the circulatory system of the organism.
