Systems and Methods for Determining Optimal Atrioventricular Pacing Delays Based on Cardiomechanical Delays

Techniques are provided for use with implantable medical devices such as pacemakers for optimizing atrioventricular (AV) pacing delays for use with cardiac resynchronization therapy (CRT). In one example, the end of atrial mechanical contraction and the onset of isovolumic ventricular mechanical contraction are detected within a patient in which the device is implanted based on cardiomechanical signals, such as cardiogenic impedance (Z) signals, S1 heart sounds or left atrial pressure (LAP) signals. Then, a cardiomechanical time delay (MC_AV) between the end of atrial contraction and the onset of isovolumic ventricular contraction is determined. AV pacing delays are set based on MC_AV to align the end an atrial kick with the onset of isovolumic ventricular contraction. Thereafter, pacing is controlled based on the AV pacing delays.

A method of facilitating therapeutic neuromodulation of a heart of a patient includes positioning an electrode in a pulmonary artery, positioning a sensor in vasculature, delivering via a stimulation system first and second electrical signals of a series of electrical signals to the electrode. The second electrical signal differs from the first electrical signal by a magnitude of a first parameter of a plurality of parameters. The method includes determining via the sensor, sensor data indicative of one or more heart activity properties in response to the delivery of the series of electrical signals, and delivering a therapeutic neuromodulation signal to the pulmonary artery using selected electrical parameters. The selected electrical parameters include a selected magnitude of the first parameter. The selected magnitude of the first parameter is based at least partially on the sensor data. The therapeutic neuromodulation signal increases heart contractility.

Application of morindae officinalis oligosaccharide pentasaccharide to preparation of drug for treating myocardial ischemia and reperfusion injury

The invention discloses an application of a morindae officinalis extract and morindae officinalis oligosaccharide pentasaccharide to preparation of a drug for treating myocardial ischemia and / or reperfusion injury and promoting therapeutic angiogenesis. The myocardial ischemia and reperfusion injury belong to secondary injury brought to an organism when the hemoperfusion of myocardium is stopped or poor, i.e., after the myocardium is subjected to ischemia, hypoxia injury, circulation reinstitution and blood supply recovery. The morindae officinalis extract and the morindae officinalis oligosaccharide pentasaccharide both play a role in protecting an in-vivo rat myocardial ischemia reperfusion injury model and an in-vitro purification cultured neonatal rat myocardial cell hypoxia / reoxygenation injury model, and can be used for remarkably reducing the myocardial infarction area and the incidence rate of reperfusion arrhythmias, effectively protecting the form of a myocardial cell and relieving the injury of hypoxia / reoxygenation to the myocardial cell. The therapeutic angiogenesis promotion means that the aims of recovering the blood supply of ischemic myocardium and improving the heart function are achieved through increasing the functional coronary artery branch or side branch under the irritation actions of some methods, including the therapeutic actions of some drugs.

An objective of the present invention is to provide methods for producing progenitor cells which show a high proliferation ability ex vivo and promote neovascularization or myocardium regeneration, by culturing pluripotent cells in a medium containing adrenomedullin (AM) and / or vascular endothelial growth factor (VEGF). Moreover, another objective of the present invention is to provide the progenitor cells and uses thereof. In order to solve the above problems, the inventors of the present application have successfully developed a novel culture technique for producing progenitor cells from adipose tissues more efficiently compared to the conventional techniques, by using adrenomedullin (AM) and / or vascular endothelial growth factor (VEGF). The progenitor cells produced by the present inventors are specialized cells having a potent proliferation ability, a revascularization ability, and a myocardium regeneration ability, and have the property of not differentiating into adipocytes even if subjected to differentiation induction into adipocytes. Therefore, the progenitor cells of the present invention can be effective pharmaceutical agents for intractable diseases in the brain, lung, liver, kidney and the like.
