A cannula drill for eccentrically adjustable bony access in percutaneous endoscopic lumbar discectomy.

This invention discloses a sleeving drill for an eccentrically adjustable percutaneous endoscopic discectomy (PED) with a bony passage. It includes a hollow sleeving into which a PED can be inserted. The end of the sleeving extending into the intervertebral foramen has an open tongue with a spiral cutting edge on its periphery. The open tongue has serrations on both sides near its root. The inner channel of the tongue is eccentrically positioned, with the cutting edge located on the thicker side. This invention, upon detecting a deviation in the bony passage, rotates the sleeving, causing the tongue to rotate as well. The spiral cutting edge then continuously corrects the direction, rotating and grinding away bone to allow entry. This effectively solves the problems of bony passage angular deviation and bone hyperplasia. Otherwise, the passage would have to be reconstructed from scratch, which is time-consuming and laborious. Because of the angular deviation in the bony passage, the difficulty of reaching the surgical site with endoscopic instruments is increased, making it difficult to completely remove nerve compression material, thus affecting the success rate of the surgery or increasing the risk of postoperative recurrence.

Spinal surgery for implanting an intervertebral device and associated devices and systems are described herein. Representative spinal surgery may include obtaining access to a diseased disc of a patient's spine via a minimally invasive lateral, transpedicular, transarticular, or transforaminal access pathway. Various instruments may be inserted through the trocar to (i) remove some or all of the diseased disc, (ii) expand the intervertebral disc space around the diseased disc, (iii) insert an intervertebral device into the intervertebral disc space, and / or (iv) fill the intervertebral device with a filling material. Each of the above steps may be performed through a minimally invasive access path provided by a trocar. This may minimize destruction of the patient's muscles, thereby minimizing patient pain and recovery time.
