A trephine blade (10, 56) for preparing a corneal graft for endothelial keratoplasty includes an asymmetric cutting formation (20) for cutting an asymmetrically shaped notch around a peripheral edge of the graft to indicate proper orientation of the graft in endothelial keratoplasty. In some embodiments, the cutting formation (20) projects along a longitudinal direction of the blade from a cutting end of the blade and generally includes a first edge (22) for making inwardly directed cuts on the graft and another cutting edge (24) for making outwardly directed cuts on the graft to form the asymmetric notch in the graft. The first edge (22) and an inner end of the other cutting edge (24) generally intersect at a curved channel (26) of the cutting formation (20). In the described embodiments, at least one of the opposite ends of the first edge (22) and the other cutting edge (24) of the cutting formation (20) is continuous with a respective curved cutting edge (32, 34) that curves into the cutting formation.
