Wristband/cinch with label assembly business form and method

A business form particularly adapted for use in a medical or hospital environment includes in a first embodiment a wristband assembly that is readily separable from a carrier comprised of a layer of face stock and a laminate, with the wristband including a printable face stock region die cut into the face stock and a strap portion, a laminating portion, and a cinch die cut into the laminate layer. The laminating portion includes two halves which fold together about a fold line to enclose the face stock, with the strap portion extending from one of said halves. The cinch may be located in one of two locations, either in an extension from the other side of the face stock or intermediate the face stock and the strap portion. In either case the cinch comprises a slot through which the strap portion is inserted and then adhered to itself after its length has been adjusted by the medical professional. If an extension is provided, it may be folded over to adhere to the strap portion and clamp it in place there as well as being adhered at its end back onto itself, as described. An extender is also described which includes a clamshell joinder and a tail portion. The clamshell portion may be applied to the strap portion anywhere along its length to extend the effective length of the strap portion. Any of the wristband constructions may be provided on a page sized sheet along with a plurality of self adhering labels, in any of a number of configurations, to suit any particular application, as desired by a user.

Wristband/label assembly business form and method

A business form particularly adapted for use during medical emergencies includes in a first embodiment a wristband/label assembly that is readily separable from a carrier, with the wristband including a single end for looping around a victim's appendage through a cinch and a tab carrying a plurality of labels with the wristband and each label having an identifying indicia such as a bar code printed thereon. The wristband bar code thus becomes associated with the victim and the labels are used to identify items associated with the patient such as his possessions, medical charts, medicines, etc. The wristband may be color coded so that as the medical personnel triage victims they are categorized by color as to their need for medical care, with the color coding thus being readily ascertainable by others as multiple victims are processed. A second embodiment includes a pre-printed form having a tab portion with the bar code labels as in the first embodiment and also a series of tear off tabs for indicating the medical condition of the patient. Additionally, the medical condition tabs may also be bar coded so that the patient's ID and medical condition may both be “swiped” into a data base using bar code information. Once the data is collected, it is conveniently input into a computer with the computer then transmitting the information to a server for display at a web site. The server and related software is fully capable of handling input from multiple computers in real time so that victim information is made available over the internet almost immediately as the victims are processed.

System and Method for Providing Body Sway Feedback to a Body of a Subject

The present invention relates to a system (10) and to a method for providing body sway feedback to a body of a subject. The system (10) comprises (a) at least one body sway sensor means (12) adapted to be attached the body and adapted for providing body sway signals indicative of a sway of the body, (b) a system processor means (14) adapted for deriving body sway information from the body sway signals and adapted for generating a body sway feedback signal from the body sway information, and (c) a subject body sway feedback means (40) having at least one transducer unit (42, 44) and adapted for providing a body sway feedback to the body via said at least one transducer unit (42, 44) based on said body sway feedback signal, (d) wherein said at least one transducer unit (42, 44) is or comprises at least one transducer, (e) wherein said at least one transducer is adapted to be contacted to a head of the body, adapted for vibrating, and adapted for transferring a respective vibration to the head, and (f) wherein said at least one transducer is adapted to evoke based on the vibration a multimodal body sway feedback as stimuli to the subject having a plurality of kinds of stimuli of the group comprising auditory stimuli, tactile stimuli, and vestibular stimuli.

Refuse vacuum system for machine shops

A vacuum system for machine shop dust, particulate matter, and the like. Blast gates control the flow of vacuum generated by a dust collector to individual machines that may or may not accommodate vacuum ductwork and the like. The blast gate has a rotating blade inside a chassis with a DC motor effectively using a slip clutch or an increased current feedback system to limit the rotation driving of the rotating blade. Importantly, a small aperture serves to allow air flow into the chassis gap through which the rotating blade rotates in order to ensure the particular matter does not lodge inside the chassis thereby preventing the proper operation of the rotating blade. A central controller having a microprocessor or the like may be programmed or programmable and serve to individually control a selected number of blast gates based upon a feedback mechanism. A piezoelectric sensor or the like senses the vibration arising from the turning on of the associated machinery, indicating the demand for vacuum power to collect particulate matter. In response to the piezoelectric sensor's signal, the blast gate is opened and the dust collector is either turned on or stepped up to accommodate the increase load upon the vacuum system. The reverse situation occurs when the machinery is turned off. The piezoelectric sensor ceases signalling the central controller of the machine "on" condition. The central controller then steps down the dust collector / vacuum system and closes the associated blast gate. Alternative embodiments are set forth whereby additional features provide for enhanced utility, including a computer operation, control, monitoring, and feedback.
