The implantation tool has inner and outer concentric tubes, the inner tube releasably threadably connected to the prosthesis, the outer tube reverse threaded with the inner tube to advance a distal headpiece to engage, deploy and lock the spikes into the deployed configuration. A vacuum assist can be used to help impact the lumen wall on the spikes. The extraction tool is similar to the implantation tool with an inner tube for threadably engaging the prosthesis, an outer tube with a distal crown with a plurality of shoes to unseat and unlock the spikes, and an overtube shield to receive the spikes and facilitate extraction of the prosthesis.

Continent ostomy port

A continent ostomy port device has a face plate defining an aperture alignable with the opening of a stoma in the user's body and a closure adjacent to the aperture is adapted to permit covering and uncovering of the aperture in the face plate. A catheter extends from one side of the face plate proximally, and one end of the catheter is disposed within the ostomy site when the port device is in use. The catheter has continuous exterior and interior side walls, the latter defining a major lumen and is sized and shaped for non-surgical insertion through a stoma to a sufficient distance that the presence of the catheter within the stoma provides a barrier which reduces the incidence of prolapse, without the use of extraneous, externally applied materials or additional surgery. A removable cartridge fits snugly and slideably within the major lumen of the catheter of the device so as to prevent inadvertent escape of body waste material from the stoma when the cartridge is in place, without use of an ostomy bag, and to clean the interior side wall of the catheter as the cartridge is pressed into the major lumen. An anti-reflux valve is activated to prevent escape of body waste and deactivated for passage of fluid. Retaining structure is connected to the catheter, and is non-surgically, snugly fittable into the stoma, to cause the port device to be self-retaining in a normal use position within a stoma, without surgery or fixation materials.

A catheter portion extends from one side of the face plate and extends proximally, and one end of the catheter portion is disposed interior of the user's body, within the ostomy site, when the port device is in normal use position. The catheter portion is sized and shaped appropriately for non-surgical insertion through a stoma to a sufficient distance that the presence of the catheter portion within the stoma provides a physical barrier which reduces the incidence of stoma prolapse, without the use of extraneous, externally applied materials or additional surgery. A removable cartridge is sized and shaped to fit snugly and slideably within the major lumen of the catheter portion of the device so as to be liquid-tight and to thereby prevent inadvertent escape of body waste material from the stoma through the device when the cartridge is in place, so that the user is not required to wear an ostomy bag, and to further thereby clean the interior side wall of the catheter portion as the cartridge is pressed into the major lumen of the catheter. A selectively operable anti-reflux valve that is attached internally of the proximal end of the catheter portion and is activated when it is desired to prevent escape of body waste through the port device, and deactivated when it is desired to permit passage of fluid through the port device. Retaining structure is connected to the catheter, and is non-surgically, snugly fittable into the stoma, to cause the port device to be self-retaining in a normal use position within a stoma of the user, without the need for special surgery and extraneous, external fixation materials such as tape, belts, and adhesives.

A plurality of induction valve elements may be joined or “ganged” together in order to define a manifold. In a specific application, a sliding sleeve valve is held away from a circulating port to allow drilling mud to be circulated out. The cylinder activating circuit also includes a return flow path and a return valve positioned in the return flow path and configured to control flow from the cylinder rod end to the fluid reservoir. Both the activation valve and the return valve are controllable by the controller independently from the regeneration flow valve.

Devices, systems, and methods for treating gastro-esophageal reflux disease (GERD) include anti-reflux valves and retainers for securing them within the lumen of the esophagus, stomach, or a hiatal hernia. The retainers contain inflatable balloons, some of which may be enveloped by a flexible shell that is used to secure the balloon to tissue. Devices with more than one balloon, a low-profile leaf valve, and a sleeve valve are used to treat reflux in patients with a hiatal hernia. The leaf valve may also be configured for its control by constriction by gastrointestinal lumen muscle, through radial compression of the valve's flange. Bi-directional combination valves are used to treat or diagnose patients with reflux due to transient lower esophageal sphincter relaxations (TLESR). Those valves pass reflux through a channel in which gas and liquid reflux are separated. Methods are described for treating GERD patients who (1) have no hiatal hernia, (2) have a hiatal hernia that is fixed in place, or (3) have a hiatal hernia that slides above and below the diaphragm. Methods are also described for delivering GERD-treatment devices to their target locations within the patient's gastrointestinal tract.
