Method and system for patient monitoring and respiratory assistance control through mechanical ventilation by the use of deterministic protocols

InactiveUS6148814AReducing ventilator rateRelieve pressureRespiratorsBreathing masksDiseaseClinical staff
A method and system for managing mechanical ventilation of patients with respiratory disorders is described. The main objective of the system is to generate executable instructions for patient care which take into account a large number of parameters of patient condition and ventilation. Data regarding the state of the patient are stored in a database. Patient data are processed according to a set of protocols which contain rules for patient care decisions arranged in a logical sequence to generate detailed, executable instructions for patient care. Instructions are updated when new data are entered into the database. The data can be acquired in an automated fashion, or the clinician can be instructed to collect and enter new data into the clinical database. Likewise, patient care instructions can be carried out automatically or manually, but it is preferred that instructions are carried out manually as a safety check. The preferred embodiment of the invention includes a computer system, software for processing patient data, and a display device for presenting patient care instructions to the clinician. The system maintains a record of patient data, patient care instructions, whether instructions were followed by the clinical staff, and if not, a reason why.

Uterine cervical cancer computer-aided-diagnosis (CAD)

Uterine cervical cancer Computer-Aided-Diagnosis (CAD) according to this invention consists of a core processing system that automatically analyses data acquired from the uterine cervix and provides tissue and patient diagnosis, as well as adequacy of the examination. The data can include, but is not limited to, color still images or video, reflectance and fluorescence multi-spectral or hyper-spectral imagery, coherent optical tomography imagery, and impedance measurements, taken with and without the use of contrast agents like 3-5% acetic acid, Lugol's iodine, or 5-aminolevulinic acid. The core processing system is based on an open, modular, and feature-based architecture, designed for multi-data, multi-sensor, and multi-feature fusion. The core processing system can be embedded in different CAD system realizations. For example: A CAD system for cervical cancer screening could in a very simple version consist of a hand-held device that only acquires one digital RGB image of the uterine cervix after application of 3-5% acetic acid and provides automatically a patient diagnosis. A CAD system used as a colposcopy adjunct could provide all functions that are related to colposcopy and that can be provided by a computer, from automation of the clinical workflow to automated patient diagnosis and treatment recommendation.

Mobile clinical workstation

A mobile workstation includes an adjustable-height horizontal tray and a pull-out keyboard tray mounted beneath the horizontal tray to support a keyboard. A device, such as computer terminal, mounts to the horizontal tray through installation into a vertically-mounted docking station mounted to the horizontal tray, or through a mounting bracket mounted adjacent to the horizontal tray. A tiltable bracket mounts the docking station to the horizontal tray, and a clutch assembly allows the angle of the bracket and the angle of the docking station and the computer terminal, to be adjusted to avoid glare on the terminal's display screen. With a mounting bracket, a tiltable bracket mounts a vertically-mounted display screen to the top of the horizontal tray. The mobile workstation also carries a power unit including an extended-life battery and a battery charger that connects to an AC power supply to charge the conventional battery pack connected to the computer terminal as well as the extended-life battery carried on the mobile workstation. When the computer terminal is stored in the mobile workstation, the wireless computer terminal can communicate through a radio transceiver in communication with a computer network, such a distributed patient-care computer network. Furthermore, a mobile workstation with a docking station includes a spring-mounted vertically-movable leveling tray that allows the computer terminal to be easily removed from the docking station using a push-down-and-tilt motion. An electronic and / or key lock selectively prevents vertical movement of the leveling tray to secure the computer terminal within the docking station.

System-wide probabilistic alerting and activation

Systems, methods, and computer program products that enable system-wide probabilistic forecasting, alerting, optimizing and activating resources in the delivery of care to address both immediate (near real-time) conditions as well as probabilistic forecasted operational states of the system over an interval that is selectable from the current time to minutes, hours and coming days or weeks ahead are provided. There are multiple probabilistic future states that are implemented in these different time intervals and these may be implemented concurrently for an instant in time control, near term, and long term. Those forecasts along with their optimized control of hospital capacity may be independently calculated and optimized, such as for a dynamic workflow direction over the next hour and also a patient's stay over a period of days. In the present application, a probabilistic and conditional workflow reasoning system enabling complex team-based decisions that improve capacity, satisfaction, and safety is provided. A means to consume user(s) judgment, implement control on specific resource assignments and tasks in a clinical workflow is enabled, as is the dynamical and optimal control of the other care delivery assets being managed by the system so as to more probably achieve operating criteria such as throughput, waiting and schedule risk.

Anaesthesia monitoring device in ultrasonic guide operation

The invention relates to an ultrasonic induction surgery center which can be used by a single hand and an anesthesia monitoring device. The ultrasonic induction surgery center and the anesthesia monitoring device comprise an ultrasonic probe (1) and a hand-hold part (4), wherein, the ultrasonic probe (1) is positioned at the front end of the hand-hold part (4); the ultrasonic induction surgery center and the anesthesia monitoring device are characterized in that a display screen (5) of ultrasonic equipment is arranged at the rear end of the hand-hold part (4). The ultrasonic probe and the display screen or an integrated design of a miniaturized type-B ultrasonic help the doctors to carry out the operation and the observation in a way that the hands and the eyes are along the same direction when in actual use, more particularly, when being applied in the motion coordination such as anesthesia and paracentesis, etc. as well as in monitoring with extremely high requirements on the precision, so as to ensure the doctor to observe the ultrasound pattern more conveniently and directly; the doctor can directly observe the operational motion of the hands and the ultrasound real time imaging within the range of observation of the visual field right ahead, thereby being beneficial to the operation of the doctor, reducing the possibility of having misoperation, greatly improving the success rate of surgery in clinical work and reducing the incidence rate and the mortality rate of complicating disease of patients.

Method and kit for detecting five psychotropic drugs and main metabolites thereof in blood

The invention belongs to the field of drug detection, and particularly relates to a method and a kit for detecting five psychotropic drugs and main metabolites thereof in blood. The five psychotropicdrugs and the main metabolites thereof comprise: olanzapine and demethyl olanzapine, risperidone and 9-hydroxy risperidone, aripiprazole and dehydrogenated aripiprazole, Escitalopram and demethyl citalopram, sertraline and N-demethyl sertraline. Accoridng to the method provided by the invention, a pair of quantitative ion pairs is respectively selected for each detection substance, a relative retention time thereof is used as a qualitative basis, and a standard curve is made by using a standard product for quantification; furthermore, the accuracy and effectiveness of the method are evaluatedfrom quality control of three low, middle and high levels, thereby avoiding distortion of the detection result; and meanwhile, an internal standard working solution is applied to correction, so that matrix effects can be avoided, and accurate quantification is realized. The method provided by the invention has the advantages of simple and rapid operation, high flux and low cost, and can be appliedto the therapeutic drug monitoring of the psychotropic drugs in the clinical work of the psychiatry department.
