Intravascular system for occluded blood vessels and guidewire for use therein

A system and method for opening a lumen in an occluded blood vessel, e.g., a coronary bypass graft, of a living being. The system comprises an atherectomy catheter having a working head, e.g., a rotary impacting impeller, and a debris extraction sub-system. The atherectomy catheter is located within a guide catheter. The working head is arranged to operate on, e.g., impact, the occlusive material in the occluded vessel to open a lumen therein, whereupon some debris may be produced. The debris extraction sub-system introduces an infusate liquid at a first flow rate adjacent the working head and withdraws that liquid and some blood at a second and higher flow rate, through the guide catheter to create a differential flow adjacent the working head, whereupon the debris is withdrawn in the infusate liquid and blood for collection outside the being's body. The introduction of the infusate liquid may also be used to establish an unbalanced flow adjacent the working head to enable the atherectomy catheter to be steered hydrodynamically. A guide wire having an inflatable balloon on its distal end may be used with the atherectomy catheter to block the flow of debris distally, while enabling distal tissues to be perfused with an oxygenating liquid. At least one flow control port may be provided in the guide catheter to prevent collapse of the vessel being revascularized. A cradle is provided to fix the guide catheter and guide wire in position within the body of the being while enabling the atherectomy catheter to be advanced along the guide wire and through the guide catheter. The guide catheter includes a wear resistant coating and is constructed so that its distal end includes plural sections of different outside diameters, with the distal most section being of the smallest outside diameter. A control console is provided to establish various modes of operation of the system based on manual inputs via switches or voice commands via voice recognition circuitry. A video panel displays the various modes of operation and instructions to the operator.

Axial force null position magnetic bearing and rotary blood pumps which use them

A generally cylindrical rotor very closely confined between two rigid thrust bearing surfaces is radially suspended by an array of attracting or repelling magnets or by a combination of permanent magnets and ring shaped members composed of ferromagnetic material. The geometry permits very small spacing between magnetic components to achieve high radial stiffness. High magnetic axial forces exerted between the rotor and stationary component on one end of the rotor are counter-balanced by equal and opposite forces at the other end of the rotor. Precise positioning of the rotor in the location where the opposing axial magnetic forces counterballance each other yields a net magnetic axial force on the rotor of near zero, hence the reference to this as the null position. Wear resistant mechanical thrust bearings confine the rotor axially to maintain this position during rotatioin. Precisely balance the magnetic axial forces in the proper geometry with relation to the mechanical thrust bearings. Blood pumps utilizing this type of bearing are disclosed, including both axial flow pump and centrifugal flow pump configurations with high flow washing of the junction of the rotating and stationary parts to prevent thrombus accumulation.

A low friction top coat over a multilayer metal / ceramic bondcoat provides a conductive substrate, such as a rotary tool, with wear resistance and corrosion resistance. The top coat further provides low friction and anti-stickiness as well as high compressive stress. The high compressive stress provided by the top coat protects against degradation of the tool due to abrasion and torsional and cyclic fatigue. Substrate temperature is strictly controlled during the coating process to preserve the bulk properties of the substrate and the coating. The described coating process is particularly useful when applied to shape memory alloys.

Device for mounting a component exposed to a pressurized fluid

The mounting of a component subjected to pressure from a fluid requires special precautions if the component is made of wear resistant, hard and hence generally brittle material and may be destroyed by locally raised stresses. A fluidic component of this kind, made, for example, of silicon / glass is arranged in an elastomeric shaped component, made, for example, of silicon rubber. The inner contours of the elastomeric shaped component correspond to the outer contours of the fluidic component. The outer contours of the shaped component corresponds to the inner contours of a holder. Due to this "floating mounting" there are no unacceptable local pressure peaks and no deformation of the fluidic component. The device is particularly suitable for mounting a fluidic component made of glass or silicon, or miniature dimensions, subject to high pressure.

Blood pump bearings with separated contact surfaces

Rotary hydrodynamic blood pumps have been used to treat over a thousand patients. The Jarvik 2000 has supported a patient for seven years and uses blood immersed bearings washed by high flow to avoid excessive thrombus formation. This permits the pump to be very simple and small. Nonetheless, the present Jarvik 2000 bearings and all other mechanical blood immersed bearings of the prior art have a supporting structure that predisposes to thrombus adjacent to the bearings. The present invention provides a bearing structure that eliminates this predilection site for thrombus formation, and may provide indefinite thrombus free operation. The rotor of the preferred embodiment includes a tapered hub fabricated of wear resistant material supported by three posts at each end of the rotor, upon which the rotor rotates. Blood washes the unobstructed spaces between the posts to prevent the accumulation of a torus of thrombus that could enlarge excessively.
