Method and apparatus for detecting vulnerable atherosclerotic plaque

Methods and devices are disclosed for detecting vulnerable atherosclerotic plaque, or plaque at risk of reducing blood flow in a vessel, by identifying a region of elevated temperature along a living vessel wall. The disclosure that human atherosclerotic plaque with measurable temperature heterogeneity has the morphological characteristics of plaque that is likely to ulcerate provides a new and sensitive technique for detecting and treating these dangerous plaques before myocardial infarction and its consequences occur. The disclosed methods are advantageous over conventional plaque detection techniques because they are capable of differentiating between those plaques that are at great risk of rupture, fissure, or ulceration, and consequent thrombosis and occlusion of the artery, and those that are not presently at risk. Infrared heat-sensing catheters useful for identifying potentially fatal arterial plaques in patients with disease of the coronary or other arteries are also described. In some embodiments a coherent infrared fiber optic bundle is employed to radially and longitudinally explore a luminal wall to identify inflamed, heat-producing, atherosclerotic plaque. Certain other methods and devices are disclosed which are particularly suited for non-invasively identifying and then monitoring the progression or amelioration of an inflamed plaque in a patient, and for monitoring for onset of inflammation in an implanted arteriovenous graft. Also disclosed are thermocouple basket catheters and thermistor basket catheters which are also capable of detecting temperature heterogeneity along a vessel wall.

The invention relates to the use of Epidermal Growth Factor (EGF) in a preferably-injectable pharmaceutical composition which is administered by means of infiltration into and around chronic cutaneous ischaemic lesions in order to prevent diabetic foot amputation. Said composition can be administered to recently-created surgical surfaces damaged by the effect of acute reperfusion with oxygenated blood following prolonged ischaemia, thereby preventing further surgical procedures and favouring the preservation of the extremity. The aforementioned composition can be used to improve (i) the cell microenvironment, thereby increasing the reparative and defensive capacity and viability of the is tissues and (ii) the cicatrisation of cutaneous ischaemic lesions, thereby stimulating cell proliferation. The invention is suitable for use in human, veterinary and experimental medicine, specifically in vascular angiology and surgery, dermatology, burn treatment and reconstructive surgery and geriatric medicine. Said composition can be used for recalcitrant ulcers which are associated with lesions in the macro and/or microvasculature, patients with inadequate lymphatic and/or venous return and ulcers or other lesions which are difficult to cicatrise and/or heal.

A heel supporting and protective boot for bed-ridden patients who may otherwise develop pressure sores on their heel. The body of the boot is comprised of a fiber and angled foam combination heretofore not used. The boot additionally has sewn tube holes on both sides allowing for use of medical tubing such as with a sequential compression pump. The inner portion material of the boot is a soft liner fabric which is antibacterial and absorbs moisture. The outer portion material of the boot is a fabric with fluid resistance. Further, the boot has a soft fiber-filled flap covering the shin portion of the leg to prevent pressure on the dorsal side of the lower leg. The boot additionally has a foot-position strap sewn and placed to the bottom of the boot running along both sides of the boot and attaching at the top of both sides of the upper boot to assist with the prevention of foot drop. Said invention further uses an opening at the heel which allows the heel to remain exposed to air as a result of proper elevation and cushioning created by the unique combination of fiber and angled foam in the body of said device. The elements incorporated address important factors in preventing or allowing healing of heel sores, or enhancing the curing of existing ulcers while minimizing chances of foot drop and minimizing chances of new pressure points. The fiber/foam combination, dual fabric content, shin flap, tubing holes, foot position strap and open heel design all work together to ensure proper healing and prevention of decubitis ulcers.
