Apparatus and methods for rapid tissue crossing are described utilizing a device to penetrate and rapidly cross a tissue layer in a patient body without the need to withdraw the tissue visualization catheter out of the patient body to be replaced with a separate dilator. The distal end of a dilator sheath, within which the visualization device is positionable, may be collapsible to form a conical dilator. Upon the placement of the tip of the dilator at the site of transseptal puncture, the conical dilator may be advanced distally through the puncture to enlarge the opening. With passage of the dilator sheath through the opening, a visualization hood may be advanced and deployed through the conical dilator which opens from its conical shape to allow the passage of the hood or other instruments therethrough.

A detectable pavement warning assembly includes a web having a plurality of projections attached to one another by linear connectors, a planar cover with a series of apertures for receiving the projections therethrough and an adhesive sheet. In use a rigid substrate such as a sidewalk is selected, and a primer is applied. The web, cover and adhesive sheet all pre-assembled are placed on the primed substrate. Next, the warning assembly is heated with an open flame to soften and fuse the pavement warning assembly with the primed substrate. The projections will remain extended sufficiently above the cover to provide a warning to pedestrians that walk thereon, such as when approaching a traffic area or other hazard.

The present invention provides a torsional energy control assembly and method for eliminating slip-stick and / or drill bit oscillations comprising axial and / or rotational oscillations. In one preferred embodiment, the assembly permits slippage between an upper portion of the drilling string and a lower portion of a drill string. The rotational control assembly may be installed at any desired position in the drill string. The rotational control assembly could also be utilized as a component of other drilling mechanisms such as a downhole drilling motor. The rotational control permits slippage while drilling for a selected time or selected rotational distance or other criteria to thereby release torsional energy in the drilling string which otherwise may produce damaging slip-stick torsional oscillations such as slip-stick. The rotational control assembly may, in one embodiment, comprise an on-off clutch whereby torque is either substantially completely transmitted or substantially not transmitted through the assembly for brief periods.

Nanoparticle formulations with enhanced mucosal penetration

Hypotonic formulations were evaluated for delivering water-soluble drugs and for drug delivery with muco-inert (that is, non-adhesive) mucus-penetrating nanoparticles (MPP). Hypotonic formulations markedly increased the rate at which drugs and MPP reached the epithelial surface, including deep into the vaginal folds. Minimally hypotonic formulations, preferably ranging from 20-220 mOsm / kg, provided rapid and uniform delivery of MPP to the entire vaginal surface, with minimal risk of epithelial toxicity. Data also show that there is a higher osmolality in the colon, such that vehicles with an osmolality above that of blood plasma (generally considered isotonic at ˜300 mOsm / kg), still lead to improvements in distribution in the colon due to rapid, osmotically-induced fluid absorption. The range for improved colon distribution with a hypotonic vehicle in the colon is ˜20 mOsm / kg-450 mOsm / kg.
