A device for excluding the left atrial appendage (LAA) includes first and second members, each with first and second ends. The second ends are connected by a hinge. The first ends interlock to close the device about the LAA. The members are shaped such that they can exclude the LAA without causing the development of necrotic tissue. If desired, a flexible cover can be provided and the first and second members can be disposed within the cover. If such a cover is used, it will contact the LAA in order to cushion the contact between the members and the LAA and minimize the tendency of the LAA to bleed. The invention also includes methods for using the device to exclude the LAA.

A method and apparatus for treating in situ biologic tissue include identifying a patient with a condition susceptible to treatment by forming a lesion in the tissue and accessing a surface of the tissue. A lesion formation tool is positioned against the accessed surface. The tool includes an optical fiber for guiding a coherent waveform of a selected wavelength to a fiber tip for discharge of light energy from the fiber tip. The wavelength is selected for the light energy to penetrate a full thickness of the tissue to form a volume of necrosed tissue through the thickness of the tissue. The tool further includes a guide tip coupled to the fiber tip. The guide tip is adapted to have a discharge bore aligned with the fiber tip to define an unobstructed light pathway from the fiber tip to the tissue surface. The guide tip is further adapted to be placed against the tissue surface with the guide tip slidable along the tissue surface in atraumatic sliding engagement with the discharge bore opposing the tissue surface. The lesion formation tool is manipulated to draw the guide tip over the tissue surface in a pathway while maintaining the discharge bore opposing the tissue surface to form a transmural lesion in the tissue extending a length of the pathway.

Device and methods for the treatment of wounds using ultrasound energy are disclosed. The portable wound treatment device may deliver ultrasound energy to a wound through direct contact with the ultrasound tip and/or through a liquid coupling medium. Several ultrasound tips specially designed to concentrate and focus ultrasound energy onto a wound are also disclosed. The ultrasound tip may also possess an abrasive peripheral boundary to aid in debriding the wound and/or removing necrotic tissue. The disclosed invention may have multiple beneficial effects in treating a wound such as sterilizing a wound, reducing external bleeding, and/or providing pain relief.

Topical exposure of nitric oxide gas to wounds such as chronic non-healing wounds may be beneficial in promoting healing of the wound and in preparing the wound bed for further treatment and recovery. Nitric oxide gas may be used, for example, to reduce the microbial infection and burden on these wounds, manage exudate secretion by reducing inflammation, upregulate expression of endogenous collagenase to locally debride the wound, and regulate the formation of collagen. High concentration of nitric oxide ranging from about 160 to 400 ppm may be used without inducing toxicity in the healthy cells around a wound site. Additionally, exposure to the high concentration for a first treatment period reduces the microbial burden and inflammation at the wound site and increase collagenase expression to debride necrotic tissue at the wound site. After a first treatment period with high concentration of nitric oxide, a second treatment period at a lower concentration of nitric oxide preferably ranging from about 5-20 ppm may to provided to restore the balance of nitric oxide and induce collagen expression to aid in the closure of the wound.

The disclosed embodiments include tools for aiding a user in debriding necrotic tissue. For example, in one embodiment, a debridement tool is disclosed that includes a housing having an interior space. A motor and a power source are disposed within the interior space of the housing. The power source is operable to provide power to the motor. A blade is coupled to a first end of the housing. The blade is operable to vibrate when the motor is powered. The debridement tool further includes a lighting element disposed within the housing and beneath a cutting edge of the blade. The lighting element is operable to illuminate the tissue site. • 4) purified HMGB1, HMGB2, and HMGB3 promote the migration of mesenchymal stem cells isolated and cultured from bone marrow;
• 5) activators containing HMGB1 which allows the migration of bone marrow mesenchymal stem cells can be conveniently purified from several organ extracts including skin, brain, and heart;
• 6) activators which allow the migration of bone marrow mesenchymal stem cells can be conveniently extracted from cultured cells; and
• 7) a heparin-column purified fraction of skin extract mobilizes a large amount of bone marrow-derived cells in case of brain injury.

According to the hydrogel skin wound dressing disclosed by the invention, Chinese herbal medicines which have the functions of resisting bacteria, diminishing inflammation, promotingtissue regeneration and removing necrotic tissues and the natural moisturizing and healing promoting materials are compounded and are in combined use to have a synergistic effect, so that the compatibility of the Chinese herbal medicines and the natural moisturizing and healing promoting materials with a hydrogel basal body is improved; the hydrogel skin wound dressing is slowly released and deeply infiltrates into skin, so that absorption of medicines and nutrient contents is promoted, and the healing of wounds is accelerated; besides, the hydrogel dressing disclosed by the invention has favorable bacterium resistance, high water absorption, favorable transparency and favorable healing promoting effects; the product is pure, and the technology is simple and convenient; the hydrogel skin wound dressing is especially suitable for wound healing of the skin, and can effectively promote healing of a II degree burn wound.
