A system is provided for collecting and storing immunization and disease data. Immunization mobile stations (IMSs) are provided with a software application to facilitate the collection of patient information such as biographical data, previous vaccination data, medical history, medications in use, occupation, administration of recent vaccination, disease symptoms and the like.
